Rituximab은 최초로 승인된 단클론 항체 항암치료제로, 국내에 서는 재발성 또는 화학요법 내성 여포형 림프종(follicular lymphoma)인 B세포 비호지킨 림프종(non-Hodgkin's lymphoma), CD20 양성의 광범위 큰 B세포 림프종(diffuse large B cell lymphoma, DLBCL), 만성림프구성백혈병(chronic lymphocytic leukemia) 등 에 적응증이 있고, 악성종양질환 외에도 류마티스성관절염, 베게너 육아종증, 현미경적 다발혈관염 등의 면역질환 치료에도 적응증을 받아 사용 중이다. 1 Rituximab은 주입관련 반응(infusion-related reaction)이 빈번 하게 나타나는데, 1 대부분이 첫 번째 투여 중에 나타난다. 2 낮은 빈 도로 지연성과민반응이 나타나기도 하는데, 실제로 병원 입원 환자 대상으로 한 후향적 연구에서 rituximab의 과민반응 중 8%가 지연 성으로 나타났다는 보고가 있다. 3

Infusion-related hypersensitivity reactions to rituximab is well known, and delayed hypersensitivity reactions to rituximab are also reported. Desensitization is commonly used to prevent immediate hypersensitivity reactions, but recently there have been cases of successful desensitization therapy for delayed hypersensitivity reactions. A 66-year-old patient who underwent rituximab treatment for diffuse large B-cell lymphoma showed repeated rituximab-induced delayed hypersensitivity reactions with whole body rashes. Intravenous rapid desensitization was performed by using a 1-bottle, 11-step protocol for 6 cycles and thereafter hypersensitivity reaction did not recur.
