A 66-yearold man presented with aphasia and right hemiparesis. Diffusion-weighted images of magnetic resonance imaging (MRI) showed a high signal intensity area in the left temporal cortex area. MR angiography showed distal M1 stenosis. After admission, transthoracic echocardiography showed a round motionless mass filling the sinus of

After 3 months, the mass at the sinus of Valsalva disappeared by the anticoagulant therapy. Therefore, we concluded the mass was thrombus. The patient had no history of any systemic disease to cause coagulation abnormalities. This is the first case of spontaneous occurrence of cerebral infarction caused by the embolism of thrombus in the sinus of Valsalva. 37:341 Valsalva (Jpn J Stroke 37: 337-341, 2015)
