Sonoelastographische Beurteilung von gesunden und posttraumatisch veränderten Achillessehnen

BACKGROUND: This study is mainly concerned with the question whether posttraumatically altered Achilles tendons differ in their long-term biomechanical properties from healthy tendons. Further associations between sonoelastographic, clinical and subjective outcome, as well as various other influencing factors were investigated. METHODS: 64 cases (affected leg) and 55 control patients (healthy leg), who were treated in the years 2004-2014 due to an Achilles tendon rupture at the University
more » ... he University Hospital Regensburg, as well as 36 healthy, young normal collective patients were followed up. The ultrasound examinations were performed with Siemens ACUSON S2000 ™ according to a standardized examination protocol, as well as the clinical outcome. The appraisal system was according to Tan et al. 2011. The subjective outcome was determined using the FAOS score and the VISA-A score. RESULTS: Strain elastographic and ARFI imaging scores were lower in post-ruptured tendons than in the contralateral or healthy normal population (1.6 vs. 1.8 vs 2.1 p = 0.047 to p <0.001). The shear wave velocity was also lower in former ruptured tendons than in healthy or normal collective tendons (2.4 m / s vs. 2.8 m / s vs. 3.5 m / s p <0.001). Furthermore, there was no correlation between the healing period (> 24 Moante-10 years) and the elastic properties (p = 0.276 to 0.952). The degree of vascularization and elasticity correlate significantly on a low level (p = 0.012 to 0.001 with Kendal-Tau-b = 0.133 to 0.256). In addition, there was no significant correlation between the type of treatment (p = 0.096 to 0.980), sex (p = 0.806 to 0158), nicotine abuse (p = 0.907 to 0.055), clinical and subjective outcome (p = 0.257 to 0.546) and the elastic properties of the tendons. CONCLUSION: Previously ruptured tendons differ in their long-term elastic properties from the healthy opposite side and appear harder / at a lower shear rate.
