Nosocomial gram negative bacterial pneumonias develop in hospitalized and are due to changes in the bacterial flora. Colonization of the upper respiratory tract by gram negative bacilli is mediated by alterations in the surface properties of the epithelial cells. In healthy individuals a film of fibronectin covers the epithelium lining the mucosa of the mouth and oropharynx and prevents the Gram-negative bacteria from adhering to the epithelial cells.
