Intramural hematoma of the duodenum is rare and usually occurs following a blunt abdominal trauma. A case of traumatic intramural hematoma of the duodenum in a child is reported. The patient was a 6-year-old male with the chief complaints of bilious vomiting and abdominal pain after abdominal trauma. Preoperative diagnosis was considered to be duodenal obstruction and the operation was performed on the 2nd day after the onset. Intramural hematoma was found involving the duodenum from the 2nd to

Although the patient still had symptoms of duodenal obstruction, he began to allow oral intake on the 23rd p.o.d. and discharged on the 45th p.o.d. uneventfully. Recently conservative treatment is prefered to surgical treatment for this disease. However, when the most use of CT, ultrasonography, and upper GI series, where the combined use of either two methods followed by careful follow-up is important, fails to yield a remission, surgical treatment should be advocated.
