Comparison of microleakage after load cycling for nanofilled composite resin fillings with or without flowable resin lining

서론 Filler의 입자가 0.1 -100 nm인 nanofilled 레진은 nanotechnology에 의해 만들어진 새로운 복합 레진으로, microfilled 레진과 비슷한 높은 투광도와 연마성을 가지 고 광택이 오래 유지되며, 1 몇몇 hybrid 레진과 동등한 물 리적 특성과 마모저항성을 가진다. 2,3 1996년 말에 소개된 flowable 레진은 특유의 높은 흐름성을 가지며, 조작이 우 수하고, 매우 심미적인 수복물을 만들 수 있지만, hybrids 레진에 비해 내구성이 떨어지는 단점이 있다. 4 지난 10년 동안 중합수축에 의한 미세 변연 누출을 줄 이기 위해 상아질 접착제를 만들었고, nanofiller와 같은 filler를 발달시켰으며 그밖에 다양한 incremental techniques, three sited photo curing method 등 여러 노력을 했으나 변연 누출은 큰 과제로 남아 있다. 이와 더불어 충 전 시 조작 미숙 또한 변연 누출을
more » ... . 이런 문제를 해결하기 위해서 flowable 레진을 liner로 쓰기도 하는데, 미세 변연 누출을 줄일 수 있다는 점에서 많은 논란이 되고 있다. Bayne 은 접근이 어려운 제2급 와 동의 경우 liner로 사용함으로써 미세 변연 누출을 줄일 수 있다고 추천했고 5 , Huysmans도 조작이 쉬우면 미세 변 연 누출을 줄일 수 있다고 했다. 6 하지만 Anders Lindberg, Belcher M, Jain P, Wibowo G, Belli S, Benzos C 등은 미세 변 연 누출과는 상관이 없다고 하였다. 7-12 이에 본 연구는 nanofilled 레진으로 제2급 와동의 레진 충전 시 미세 변연 누출을 감소시키기 위해 nanofilled flowable 레진을 liner로 사용하는 경우와 사용하지 않는 경우에서 반복 하중 전후에 미세 변연 누출을 비교함으 로써 flowable 레진이 변연 누출에 대해 어떠한 영향을 주 는지 알아보고자 하였다. 그리고 반복 하중 후에도 liner 로 사용된 flowable 레진이 하중에 견딜 수 있는 기계적 물성이 있는지 알아보고자 하였다. Statement of problem: when using resin for class II restoration, micoleakage by instrumentation can be regarded as the primary negative characteristic. A review of the available literature suggests that using flowable resin as liner to decreased microleakage. Purpose: The aim of this study was to determine the influence of the nanofilled flowable resin lining on marginal microleakage after load cycling in class II composite restoration fillings using nanofiller resin. Material and methods: 24 extracted premolars were prepared with class II cavity. F group was restored the nanofilled resin with the nanofilled flowable resin as liner. NF group was restored the nanofilled resin only. After restoration, an experiment was performed on 2 groups using a 300N load at 104, 105 and 106 cycles. Prior to and before each load cycling, it was gauged length on total marginal microleakage, axial marginal microleakage and buccal, gingival, lingual marginal microleakage. Data were analyzed with the Mann-Whitney test & Kruskal-Wallis test. Results: There were statistically significant differences between 2 groups and between individual groups. (P < .05) The result showed less microleakage in teeth restored by the nanofilled resin, which was lined by the nanofilled flowable resin. Conclusion: There was significant reduction in microleakage when the nanofilled flowable resin lining was placed underneath the nanofilled resin in class II composite restoration fillings.
