... lth through the mediation of affecting health determinants at a local level. The community health care is provided by a multidisciplinary team of professionals. In addition to the activity of practitioners, specialists and pharmacists, the community nursing care occupies an important position in the community care. In a system based on the community care, nurses should support and maintain connections between individuals, families and communities. The community nursing care is a synthesis of the nursery practice and public health applied to supporting and saving the population health. Support of the health, maintenance of the health, health education, management, coordination and continuity of the care are employed in the holistic approach to providing the health care in individuals, families, groups and communities. Nurses and midwives are considered as the most important source of providing reform strategies associated with the transformation of hospitals by reducing the period of hospitalization and particularly with the development of the community care. Positive changes in the Czech Republic in the community health care development currently bring legislation adjustments providing conditions for the development of the new type of the special education programme: community nurses and midwives. By Regulation of the Ministry of Health of the Czech Republic 424/2004 Sb., which is presented next to Law No. 96/2004 Sb., activities of general nurses and midwives in the field of the community care are particularly specified. Souhrn Rozšíření komunitní zdravotní péče, zvláště ošetřovatelské, je klíčem ke zlepšení zdraví i tělesné a duševní pohody všech lidí. Vytváření systému péče o zdraví na komunitní úrovni je poměrně novou komponentou systému péče o zdraví. Subsystém komunitní péče o zdraví představuje základní funkční jednotku celého systému péče o zdraví s orientací především na zdraví. Soustřeďuje se na dosažení zdraví prostřednictvím ovlivňování determinant zdraví na lokální úrovni. Komunitní zdravotní péče je zajišťována multidisciplinárním týmem pracovníků. Mimo činnost praktických lékařů, specialistů a lékárníků zaujímá významné místo v komunitní péči komunitní ošetřovatelská péče. V systému založeném na komunitní péči by měly sestry podporovat a udržovat spojení mezi jednotlivci, rodinami, komunitami. Komunitní ošetřovatelství je syntézou ošetřovatelské praxe a veřejného zdraví aplikovaná na podporu a zachování zdraví populace. Podpora zdraví, udržení zdraví, zdravotní výchova, management, koordinace a kontinuita péče jsou využity v holistickém přístupu k zajištění zdravotní péče jednotlivců, rodin, skupin a komunit. Sestry a porodní asistentky jsou považovány za nejdůležitější zdroj pro zajištění reformních strategií v souvislosti s transformací nemocnic, zkracováním doby hospitalizace a především s rozvojem komunitní péče. Pozitivní změny pro Českou republiku v rozvoji komunitní zdravotní péče v současnosti přinášejí legislativní úpravy, které připravily podmínky pro rozvoj nového typu specializačního vzděláváníkomunitní sestry a porodní asistent-
