Many rheumatic diseases may involve any organ system of the body including the skin. Proper understanding of rheumatic skin disease is necessary for making a diagnosis. In addition, there are important relationships existing between the cutaneous and systemic manifestations of rheumatic diseases. MeSH 분류에 의하면 류마티스병은 근골격질환, 피부 와 결합조직병의 하위 분류로 류마티스관절염, 섬유근육 통 통풍, Sternocostoclavicular Hyperostosis, 골관절염, Polymyalgia Rheumatica, 류마티스열 등이 속하나 이 글에서는 결합조직병에 속하는 대표적인 질환인 홍반루푸스(lupus erythematosus), 피부근염(dermatomyositis)과 피부경화증 (scleroderma)의 피부 소견에 대해 설명하고자 한다. 이들 질환의 피부 소견은 전신 소견과 밀접한 연관이 있고 진단 에 내릴 때 매우 중요한 단서가 되므로, 피부 증상을 정확 하게 파악하는 것이 올바른 진단과 이에 따른 적절한 치료 의 첫 걸음이 될 수 있기 때문이다. 홍반루푸스(Lupus Erythematosus, LE) 홍반루푸스에서 피부는 두 번째로 많이 병발하는 기관으로 매우 다양하게 나타난다 (1). 홍반루푸스의 피부 소견은 1981 년 Gilliam이 제안한 분류에 의하면 특징적인 조직 소견을 보 이는 홍반루푸스 특이 피부병(LE-specific skin disease)과 조 직학적으로 특징적인 소견을 보이지 않아 다른 질환에서도 볼 수 있는 홍반루푸스 비특이 피부병(LE-nonspecific skin disease)으로 나눌 수 있다(Table 1) (2,3).
