Purpose: To report a case of sebaceous adenoma, which is a rare benign eyelid tumor.Case summary: A 45-year-old male presented with a visible mass on his left upper eyelid that had recurred 2 months ago. The mass had been resected 2 years ago. When the upper eyelid was everted, the tarsal portion of the mass was visible as a whitish papillary lesion. An incisional biopsy was done for pathological diagnosis and sebaceous adenoma was confirmed based on histopathological examinations.

Immunohistochemical studies were performed for differential diagnosis of Muir-Torre syndrome (MTS). Mutator L homologue 1, mutator S homologue 2, mutator S homologue 6 and PMS1 homolog 2 were all positive and he had no past medical or family history of internal malignancy, suggesting a very low possibility of MTS. Finally, the lesion was diagnosed as a sporadic sebaceous adenoma. The patient was followed up for 12 months without recurrence. Conclusions: Eyelid sebaceous adenoma should be considered as a differential diagnosis for an eyelid mass originating from the tarsal conjunctiva.
