We describe a case of a 65-year-old man who presented with progressive generalized weakness. He was started on daptomycin for methicillin-resistant Staphylococcus aureus (MRSA) right fifth metatarsal osteomyelitis. Laboratory testing revealed severe hyperkalemia of 7.5 mEq/L (normal range: 3.3-5.1) and acute kidney injury (AKI) stage 1 associated with a substantial elevation in serum creatine kinase (CK).
