Bilateral adrenal neoplasms are associated with metastatic cancer, pheochromocytoma and lymphoma. The coexistence of a unilateral functioning adrenocortical adenoma with contralateral pheochromocytoma is extremely rare. ... le cortisol. Computerized tomographic scanning revealed a bilateral adrenal tumor. To preserve adrenal function, right adrenalectomy along with left adrenal tumorectomy was performed. Histological finding of the right adrenal tumor was pheochromocytoma and the left adrenal tumor was adrenocortical adenoma. This patient was the first case of a functional adrenocortical adenoma with contralateral cystic pheochromocytoma in Korea. We report the case with a review of the literature. ( Endocrinol Metab 27:323-328, 2012) 양측성 부신종양은 드문 질환으로 전이성암종, 갈색세포종, 림프 종 등에서 관찰되며 동일한 종양이 양측으로 발생한 경우가 대부분 이다[1]. 양측성 부신종양의 경우 편측만이 기능을 하거나, 양측이 기능을 하더라도 한 가지 호르몬을 분비하는 경우가 많다. 양측이 서로 다른 호르몬을 분비하는 경우는 극히 드물어, 국내에선 코르 티솔과 알도스테론을 분비한 양측성 부신피질선종 1예가 보고되었 다[2]. 피질에서 기원한 선종과 수질에서 기원한 갈색세포종이 양측 에 발생한 경우는 외국에서 6예 보고되었으며[3-8], 국내에서는 아 직 보고된 바가 없다. 저자들은 낭종성 갈색세포종과 코르티솔 분 비 부신피질선종이 각기 다른 부신에서 발생한 예를 경험하였기에 문헌고찰과 함께 보고하고자 한다. 증 례 환자: 52세, 여자 주소: 피로, 두통, 심계항진 현병력: 내원 2개월 전부터 피로, 두통, 심계항진 등의 증상 있어 시행한 복부 컴퓨터단층촬영(computed tomography)에서 좌측 부 신종양, 우측 부신낭종 관찰되어 추가 검사 위해 본원 내원하였다. 과거력: 2개월 전 제2형 당뇨병 진단 받았다. 가족력: 특이 사항 없었다. 신체검진: 키 167 cm, 체중 61 kg, 체질량지수 21.9 kg/m 2 이었으며 내원 당시 혈압은 120/80 mmHg, 맥박수 60회/분, 호흡수 20회/분, 체온 36.2°C이었다. 만성병색을 보였으며, 전신무력감을 호소하였다. 2개월 사이에 6 kg의 체중 증가가 있었다. 신체검진에서 쿠싱증후군 This is an Open Access article distributed under the terms of the Creative Commons Attribution Non-Commercial License (http://creativecommons.org/licenses/by-nc/3.0/) which permits unrestricted non-commercial use, distribution, and reproduction in any medium, provided the original work is properly cited.
