... res through a median sternotomy with follow up of up to three years. Results: In all the cases we obtained optimal surgical exposure. All the wounds healed well. There were no hematomas. There was a recovery of the initial hyposthesia and the sternum healed well. The cosmetic results were at the entire satisfaction of all patients. Conclusions: The bilateral submamary insicion for median sternotomy offers excelent surgical exposure of the mediastinum and heart. RESUMEN Objetivo: Demostrar la técnica y resultados del uso de una incisión cosmética en procedimientos quirúrgicos (p.ej.: cirugía cardiaca) que requieran de esternotomía media, con la finalidad de reducir problemas de tipo sicológico en mujeres que se deban someter a esta cirugía; ya que si se realizara con una incisión dérmica vertical medioesternal dejaría una cicatriz mucho más notoria. Material y Métodos Se presentan seis casos de incisión cosmética submamaria bilateral y esternotomía media en mujeres jóvenes que se sometieron a procedimientos cardíacos y/o mediastínicos, con seguimiento de hasta tres años. Resultados: En todos los casos se obtuvo buena exposición quirúrgica, las heridas cicatrizaron bien. No hubo hematomas. No hubo mortalidad operatoria. Hubo recuperación del déficit sensitivo mamario y periareolar. El esternón consolidó bien. El resultado cosmético de la incisión fue de entera satisfacción para todas las pacientes. Conclusiones: La incisión submamaria bilateral para esternotomía media ofrece excelente exposición quirúrgica del mediastino y corazón. Su utilización es factible de realizar en nuestro medio sin agregar riesgo adicional y obteniendo un beneficio sicológico notorio en pacientes mujeres. ( Rev Med Hered 1998; 9:10-13).
