Chinese Stroke Association guidelines for clinical management of cerebrovascular disorders

The Chinese Stroke Association's Levels of Evidence grading algorithm was used to grade each recommendation.ResultsEvidence-based guidelines are presented for the organisational management of patients presenting with stroke. The focus of the guideline was subdivided into prehospital first aid system of stroke, rapid diagnosis and treatment of emergency in stroke centre, organisational management of stroke unit and stroke clinic, construction of regional collaborative network among stroke centres and evaluation and continuous improvement of stroke medical quality.ConclusionsThe guidelines offer an organisational stroke management model for patients with stroke which might help dramatically.

Genomic selection of agronomic traits in hybrid rice using an NCII population

Hybrid breeding is an effective tool to improve yield in rice, while parental selection remains the key and difficult issue. Genomic selection (GS) provides opportunities to predict the performance of hybrids before phenotypes are measured. more » ... tical methods, heritability, marker density and training population size on prediction for hybrid performance. Results: From the comparison of six GS methods, we found that predictabilities for different methods are significantly different, with genomic best linear unbiased prediction (GBLUP) and least absolute shrinkage and selection operation (LASSO) being the best, support vector machine (SVM) and partial least square (PLS) being the worst. The marker density has lower influence on predicting rice hybrid performance compared with the size of training population. Additionally, we used the 575 (115 × 5) hybrid rice as a training population to predict eight agronomic traits of all hybrids derived from 120 (115 + 5) rice varieties each mating with 3023 rice accessions from the 3000 rice genomes project (3 K RGP). Of the 362,760 potential hybrids, selection of the top 100 predicted hybrids would lead to 35.5%, 23.25%, 30.21%, 42.87%, 61. 80%, 75.83%, 19.24% and 36.12% increase in grain yield per plant, thousand-grain weight, panicle number per plant, plant height, secondary branch number, grain number per panicle, panicle length and primary branch number, respectively. Conclusions: This study evaluated the factors affecting predictabilities for hybrid prediction and demonstrated the implementation of GS to predict hybrid performance of rice. Our results suggest that GS could enable the rapid selection of superior hybrids, thus increasing the efficiency of rice hybrid breeding.

Idiopathic mesenteric phlebosclerosis: clinical and CT imaging characteristics

The aim of our study was to evaluate the clinical characteristics and computed tomography (CT) imaging findings of idiopathic mesenteric phlebosclerosis (IMP). From January 2013 to May 2019, the clinical data of 10 patients diagnosed with IMP were analyzed retrospectively. more » ... , and the surrounding fat gap. The calcification score was calculated according to the extent of the involved mesenteric veins. The colonic wall thickness was defined as the average value of the thickest and thinnest regions of the intestinal wall. The correlation between the calcification scores and the colonic wall thickness was analyzed using Spearman's correlation analysis. All 10 patients were male with an average age of 59.6 years (range, 51-83 years). The average smoking index was 712 (range, 0-1,800). Among them, 7 patients had a history of long-term excessive daily intake of medicinal liquor or Chinese herbal medicine. Clinical symptoms of abdominal pain, diarrhea, bloating, and nausea were found. Colonoscopy showed dark purple discolorations of the edematous mucosa, engorged blood vessels, extensive erosion, ulceration, and multi-focal nodular surface in all patients. CT demonstrated colonic wall thickening, calcification along the mesenteric vein, and blurry surrounding fat gap in all 10 patients. Mesenteric venous calcification involved the terminal ileum, the ascending and transverse colon in all patients, and the descending colon and sigmoid colon's involvement in two patients. A total of 33 segments of the intestinal wall were involved. The median calcification score was 6 points, the mean thickness of the colonic wall was 10.73±3.22 mm, and there was no significant correlation (P=0.782) between calcification score and thickness of the colonic wall. The main features of IMP are mesenteric venous calcification, colonic wall thickness, and pericolic fat stranding, and there is no correlation between calcification score and colonic wall thickness. Therefore, CT imaging combined with colonoscopy can improve the diagnostic accuracy of IMP.

Pituitary stalk management during the microsurgery of craniopharyngiomas

In the present study, 203 patients that had previously undergone microsurgery for craniopharyngiomas (CPs) between 1992 and 2012 were analyzed retrospectively on a long-term follow-up basis to investigate the differences in the recurrence rate and endocrine function between patients with preserved and resected pituitary stalks. more » ... e resections of the pituitary stalk. Tumor origins and the involvement of the pituitary stalk during surgery were observed. From 2010 onwards, an ultra-electron microscope was used postoperatively to detect whether pituitary stalk specimens were infiltrated or invaded with tumor cells. Long-term follow-up observations of the patients included tumor recurrence, postoperative endocrine dysfunction and visual acuity and field. Among the 203 patients, 175 patients received gross-total resection (GTR) (175/203, 86.2%), 28 patients underwent subtotal resection (28/203, 13.8%) and 34 patients had surgery that preserved the pituitary stalk (34/203, 16.7%). There was no significant difference in the recurrence rate between Group A (4/34, 11.8%) and the patients in Group B (10/123, 8.1%) who underwent GTR and also received follow-ups. Of the 157 patients who were followed up, 91 individuals underwent endocrine evaluation and the outcome was divided into normal, satisfactory and poor grades. The results for Group A were 5, 18 and 0, respectively, while the results for Group B were 1, 60 and 7, respectively, which showed a statistically significant difference between the groups. Pituitary stalk specimens of 15 patients were studied postoperatively using an ultra-electron microscope and all samples showed tumor cells had invaded the pituitary stalk (15/15, 100%).

more » ... ifferent temperature regimes. Thus, we are able to measure the optical reflectivity of the in-plane (ab-plane) thin films from room temperature to 5 K. A critical behavior near the superconducting transition temperature (T c ∼10 K) can be observed optically in the temperature-dependent reflectivity spectrum. We carry out the Kramers-Kronig analysis of the experimental data and discuss the basic optical properties of superconducting FeSe thin films in different structural phases above and below the transition temperature T s ∼100 K. By fitting experimental data with the Drude-Smith and Drude-Lorentz formulas, we obtain the key sample parameters such as the electronic relaxation time and dc conductivity. The interesting and important findings obtained from this study not only shed new light on the pairing mechanism in FeSe but also demonstrate that the NIR reflection experiment is a powerful and convenient optical technique for contactless characterization and investigation of superconducting thin film materials.

AIm: We present the long-term outcomes as well as their correlation with tumor size in 127 consecutive patients harboring large MSWM after microsurgical treatment. mATeRIAL and meTHods: The retrospective analysis of clinical data and follow-up data of 127 microsurgical treated patients with MSWM was performed. more » ... available in 120 cases for a mean duration of 81.6 months (12-216 months). The permanent morbidity was 14.2%. The mean KPS score 1 year after surgery was 90.6 (ranged 60-100). Among 74 patients of preoperative visual acuity (VA) impairment, postoperative VA improved in 42 cases (56.8%), unchanged in 30 (40.5%), and deteriorated in 2 (2.7%). MR images revealed tumor recurrence after total resection in 10 cases (10.2%) and tumor progression after subtotal resection in 10 cases (45.5%). CoNCLusIoN: Tumor recurrence was the major risk in the long run, thus the initial surgery was extremely important and hence should be aggressive. The size of tumor affected the extent of tumor removal determining clinical outcomes including VA improvement and KPS score immediately after surgery; however, it was not correlated with long-term overall outcomes. ÖZ AmAÇ: Büyük MSWM bulunan arka arkaya 127 olguda mikrocerrahi tedavi sonrasında uzun süreli sonuçları ve tümör büyüklüğüyle korelasyonlarını sunuyoruz. yÖNTem ve GeReÇLeR: MSWM bulunan ve mikrocerrahi ile tedavi edilen 127 hastanın klinik verileri ve takip verilerinin retrospektif analizi yapıldı. Tümörlerin ortalama maksimum çapı 5,2 cm (aralık 1,5 -10,0 cm) bulundu. BuLGuLAR: 104 olguda (%81,9) gros total rezeksiyon yapıldı. Operatif mortalite yoktu. Ayrıntılı takip verileri 120 olguda ortalama 81,6 ay (12-216 ay) için mevcuttu. Kalıcı morbidite %14,2 oranında görüldü. Cerrahiden 1 yıl sonra ortalama KPS skoru 90,6 (aralık 60-100) bulundu. Preoperatif görme keskinliği bozukluğu olan 74 hasta içinde postoperatif dönemde görme keskinliği 42 hastada (%56,8) arttı, 30'unda (%40,5) değişmedi ve 2'sinde (%2,7) kötüye gitti. MR görüntüleri total rezeksiyon sonrasında 10 olguda (%10,2) tümör reküransı ve subtotal rezeksiyon sonrasında 10 olguda (%45,5) tümör ilerlemesi gösterdi. soNuÇ: Uzun dönemde temel risk tümör reküransıydı ve bu nedenle ilk cerrahi çok önemlidir ve agresif olmalıdır. Tümör büyüklüğü, tümör çıkarma kapsamını etkileyerek ameliyattan hemen sonra görme keskinliği düzelmesi ve KPS skoru dahil klinik sonuçları etkiledi ancak uzun dönemli genel sonuçlarla korelasyon göstermedi. Efficacy and Safety of Adherence to dl-3-n-Butylphthalide Treatment in Patients With Non-disabling Minor Stroke and TIA—Analysis From a Nationwide, Multicenter Registry

Dl-3-n-Butylphthalide (NBP) has the potential to improve clinical outcomes in acute ischemic stroke patients by improving collateral circulation. more » ... apy in acute non-disabling ischemic stroke or high-risk TIA. Non-disabling minor ischemic stroke patients within 48 h were enrolled across 51 stroke centers in China. We divided patients into NBP compliance or non-compliance groups according to their adherence to NBP. The primary outcome was the favorable functional outcome at 90 days, defined as a modified Rankin scale (mRS) <2.Results: Between 10th October 2016 and 25th June 2019, 3,118 patients were included in this analysis. In multivariable analysis, after adjusting for common risk factors and demographic factors, NBP-compliance group has a higher proportion of favorable functional outcome (92.1 vs. 87.4%, adjusted odds ratio 2.00, 95% confidence interval, 1.50–2.65), and a higher stroke recurrence rate (2.40 vs. 0.31%, adjusted odds ratio 8.86, 95% confidence interval, 3.37–23.30) than the NBP-non-compliance group. There was no significant difference in death and intracranial hemorrhage rate between the two groups. In subgroup analysis, patients with National Institutes of Health Stroke Scale (NIHSS) scores from 3 to 5 who complied to NBP therapy had a higher rate of favorable functional outcomes than the NBP-non-compliance group. [89.8 vs. 76.8%, adjusted odds ratio 2.54 (1.64–3.93), adjusted interaction P = 0.001].Conclusion: In non-disabling minor ischemic stroke or TIA patients, compliance with NBP therapy led to better 90-day functional outcomes despite a higher risk of recurrence, and this effect seems to be stronger in patients with NIHSS scores of 3–5. Further large randomized, double-blind controlled studies to analyse the association between NBP and functional outcome is warranted in the coming future.

Single-Cell Profiling of Kidney Transplant Recipients With Immunosuppressive Treatment Reveals the Dynamic Immune Characteristics

Kidney transplantation is currently the first choice of treatment for various types of end-stage renal failure, but there are major limitations in the application of immunosuppressive protocols after kidney transplantation. more » ... pare the immune status of the recipient's whole peripheral blood before and after receipt of immunosuppressive agents, we used single-cell cytometry by time-of-flight (CyTOF) to detect the peripheral blood immune cells in five kidney transplant recipients (KTRs) from the Department of Organ Transplantation of Zhujiang Hospital of Southern Medical University before and after receiving immunosuppressive agents. Based on CyTOF analysis, we detected 363,342 live single immune cells. We found that the immune cell types of the KTRs before and after receipt of immunosuppressive agents were mainly divided into CD4+ T cells, CD8+ T cells, B cells, NK cells/γδ T cells, monocytes/macrophages, granulocytes, and dendritic cells (DCs). After further reclustering of the above cell types, it was found that the immune cell subclusters in the peripheral blood of patients underwent major changes after receipt of immunosuppressants. After receiving immunosuppressive therapy, the peripheral blood of KTRs had significantly increased levels of CD57+NK cells and significantly decreased levels of central memory CD4+ T cells, follicular helper CD4+ T cells, effector CD8+ T cells, effector memory CD8+ T cells and naive CD8+ T cells. This study used CyTOF to classify immune cells in the peripheral blood of KTRs before and after immunosuppressive treatment, further compared differences in the proportions of the main immune cell types and immune cell subgroups before and after receipt of immunosuppressants, and provided relatively accurate information for assessment and treatment strategies for KTRs.

We present initial results from the first systematic survey of luminous z∼ 5.5 quasars. Quasars at z ∼ 5.5, the post-reionization epoch, are crucial tools to explore the evolution of intergalactic medium, quasar evolution and the early super-massive black hole growth. However, it has been very challenging to select quasars at redshifts 5.3 < z < 5.7 using conventional color selections, due to their similar optical colors to late-type stars, especially M dwarfs, resulting in a glaring redshift

We develop a new selection technique for z ∼ 5.5 quasars based on optical, near-IR and mid-IR photometric data from Sloan Digital Sky Survey (SDSS), UKIRT InfraRed Deep Sky Surveys - Large Area Survey (ULAS), VISTA Hemisphere Survey (VHS) and Wide field Infrared Survey Explorer (WISE). From our pilot observations in SDSS-ULAS/VHS area, we have discovered 15 new quasars at 5.3 < z < 5.7 and 6 new lower redshift quasars, with SDSS z band magnitude brighter than 20.5. Including other two z ∼ 5.5 quasars already published in our previous work, we now construct an uniform quasar sample at 5.3 < z < 5.7 with 17 quasars in a ∼ 4800 square degree survey area. For further application in a larger survey area, we apply our selection pipeline to do a test selection by using the new wide field J band photometric data from a preliminary version of the UKIRT Hemisphere Survey (UHS). We successfully discover the first UHS selected z ∼ 5.5 quasar.
