Subarachnoid hemorrhage after transient global amnesia caused by cerebral venous congestion: case report
2018
BMC Neurology
Transient global amnesia is reported to be caused by cerebral venous congestion. Internal jugular venous flow reversal in particular with the Valsalva maneuver leads to cerebral venous congestion. In addition, Valsalva maneuver can also induce subarachnoid hemorrhage. Transient global amnesia and subarachnoid hemorrhage might have common a pathophysiology in cerebral venous congestion. Case presentation: We report here the case of a 57-year-old woman who twice experienced convexal subarachnoid

... emorrhage just after straining at stool following an episode of transient global amnesia. Digital subtraction angiography showed left temporal congestion. Left jugular vein ultrasonography revealed reflux with the Valsalva maneuver only in acute phase, indicating transient cerebral venous congestion. Conclusions: Subarachnoid hemorrhage followed by transient global amnesia indicates a common factor between them. Transient venous congestion is discussed in order to explain this rare phenomenon.
Blast mitigation by water in a bag on a tunnel floor
2018
MATEC Web of Conferences
Mitigation of blast wave, caused by explosion of explosives, from a straight tube using water in a bag (water bag) was evaluated. The length of the tube was 330 mm and the cross-section area was 30 x 30 mm 2 . One end of the tube was closed. The water bag was placed on the floor or closed end wall of the tube near the explosive. The thickness of water was 3 mm. A specially designed small detonator, which contains lead azide of 100 mg, was ignited near the closed end wall of the tube. The blast

... ressure outside the tube was measured and examined. The blast wave was remarkably mitigated by the water bag. Equivalent ratio analysis revealed that the glass beads absorbed 33%-45% of explosion energy.

Wheelchair lift which is installable both of steps and stairs
2019
Transactions of the JSME (in Japanese)
This study proposes a novel step and stair lifting system for wheelchair. In general households, it is difficult for setting the step/stair lift to provide a large space at the front of the entrance, because the size of the system is very large. In addition, the system requires an actuator separate from the lifting drive for folding the platform and wheel-stops. As the number of actuators increases, the scale of the system also increases. Therefore, a stair and step lifting system were designed

... to satisfy these concepts as follows: 1) do not occupy the space in lower floor side, 2) stair and step common system, and 3) one degree of freedom mechanism. Based on these concepts, the proposed system has only one degree of freedom for driving rail to lift platform, and can be installed at both the stairs and the step by changing the inclination of the driving rail. In this paper, the designs for the step and stair of the proposed system are described. Furthermore, mechanisms for folding a platform and mechanisms for opening and closing wheel-stops are proposed using the lifting drive. For lifting motion, the disturbance observer is integrated to the control system. Finally, the effectiveness of the proposed systems are elucidated through the experiments.

A highly sensitive trap vector system for isolating reporter cells and identification of responsive genes
2018
Biology Methods and Protocols
We devised a versatile vector system for efficient isolation of reporter cells responding to a certain condition of interest. This system combines nontoxic GAL4-UAS and piggyBac transposon systems, allowing application to mammalian cells and improved expression of a fluorescent reporter protein for cell sorting. Case studies under conditions of c-MYC gene induction or endoplasmic reticulum (ER) stress with thapsigargin on mouse or human cell lines confirmed easy and efficient isolation of

... sive reporter cells. Sequence analyses of the integrated loci of the thapsigargin-responsive clones identified responsive genes including BiP and OSBPL9. OSBPL9 is a novel ER stress-responsive gene and we confirmed that endogenous mRNA expression of OSBPL9 is upregulated by thapsigargin, and is repressed by IRE1α inhibitors, 4μ8C and toyocamycin, but not significantly by a PERK inhibitor, GSK2656157. These results demonstrate that this approach can be used to discover novel genes regulated by any stimuli without the need for microarray analysis, and that it can concomitantly produce reporter cells without identification of stimuli-responsive promoter/enhancer elements. Therefore, this system has a variety of benefits for basic and clinical research.

A cold-blooded tiptoer: non-resolving cellulitis in an immunocompromised patient
2022
Open Forum Infectious Diseases
Mycobacterium haemophilum (M. haemophilum) is a non-tuberculous mycobacteria (NTM) with a predilection for skin and soft tissue infection (SSTI) in the immunocompromised host. We report a case of disseminated M. haemophilum infection initially presenting as a non-resolving subacute cellulitis of bilateral lower extremities. Genetic sequencing was utilized for final identification, while a commercially available polymerase chain reaction test returned false positive result for Mycobacterium

A novel difficulty scoring system for laparoscopic liver resection
2014
Journal of hepato-biliary-pancreatic sciences
Early on, laparoscopic liver resection (LLR) was limited to partial resection, but major LLR is no longer rare. A difficulty scoring system is required to guide surgeons in advancing from simple to highly technical laparoscopic resections. Subjects were 90 patients who had undergone pure LLR at three medical institutions (30 patients/institution) from January 2011 to April 2014. Surgical difficulty was assessed by the operator using an index of 1-10 with the following divisions: 1-3 low

... ty, 4-6 intermediate difficulty, and 7-10 high difficulty. Weighted kappa statistic was used to calculate the concordance between the operators' and reviewers' (expert surgeon) difficulty index. Inter-rater agreement (weighted kappa statistic) between the operators' and reviewers' assessments was 0.89 with the three-level difficulty index and 0.80 with the 10-level difficulty index. A 10-level difficulty index by linear modeling based on clinical information revealed a weighted kappa statistic of 0.72 and that scored by the extent of liver resection, tumor location, tumor size, liver function, and tumor proximity to major vessels revealed a weighted kappa statistic of 0.68. We proposed a new scoring system to predict difficulty of various LLRs preoperatively. The calculated score well reflected difficulty.
