Characterization of Mucosal Dysbiosis of Early Colonic Neoplasia [article]

AbstractAberrant crypt foci (ACF) are the earliest morphologically identifiable lesion in the colon that can be detected by high-definition chromoendoscopy with contrast dye-spray. Although frequently associated with synchronous adenomas, their role in colorectal tumor development, particularly in the proximal colon, is still not clear. The goal of this study was to evaluate the profile of colon-associated bacteria associated with proximal ACF and to investigate their relationship to the
more &raquo; ... e and subtype of synchronous polyps present throughout the colon. Forty-five subjects undergoing a screening or surveillance colonoscopy were included in this retrospective study. Our study cohort included a total of 16 subjects with no identifiable proximal lesions (either ACF or polyp), 14 subjects with at least 1 ACF but no polyp(s), and 15 subjects with both at least 1 ACF and a synchronous proximal polyp(s) detected at colonoscopy. Bacterial cells adherent to the epithelia of ACF and normal mucosa were visualized by in situ hybridization within confocal sections. Bacterial DNA isolated from biopsies was used to construct PCR amplicon libraries targeting the V4 hypervariable region of the 16S rRNA gene, which were then sequenced on the Illumina platform. ACF showed significantly greater heterogeneity in their bacterial profiles compared to normal mucosa. Interestingly, one of the bacterial community structures we characterized was strongly correlated with the presence of synchronous polyps. The observed dysbiosis is more prominent within the colonic epithelium that also harbors synchronous polyps. Finally using DNA-mass spectrometry to evaluate a panel of colorectal cancer hot-spot mutations present in the ACF, we found that several APC gene mutations (R1450*, R876*, S1465fs*3) were positively associated with the presence of Instestinibacter sp., whereas KRAS mutations (G12V, G12D) were positively correlated with Ruminococcus gnavus. This result indicates a potential relationship between specific colon-associated bacterial species and somatically acquired CRC-related mutations. Overall, our findings suggest that perturbations to the normal adherent mucosal flora may constitute a risk factor for early neoplasia, demonstrating the potential impact of mucosal dysbiosis on the tissue microenvironment and behavior of ACF that may facilitate (or impede) their progression towards more advanced forms of neoplasia.

S1P1-selective agonist, SEW2871, ameliorates ischemic acute renal failure

The pathogenesis of renal ischemia/reperfusion (I/R) injury involves activating several signal transduction cascade systems in endothelial cells. Sphingosine 1-phospate (S1P) maintains endothelial cell integrity and inhibits lymphocyte egress via the specific S1P 1 receptor, and may play a role in reducing ischemic renal injury. We examined the protective effects of a newly identified S1P 1 -selective agonist, SEW2871, on mouse renal I/R injury. Kidneys were harvested 1-4 days after I/R injury
more &raquo; ... or histopathology, immunofluorescence studies, and quantitative real-time reverse transcriptasepolymerase chain reaction analyses to assess the change in gene expression profiles of inflammation-associated cytokines and adhesion molecules. SEW2871 improved renal function with a 40% reduction in plasma creatinine levels (Po0.01) and a significant reduction in tubular necrosis scores (I/R only: 4.370.2 vs I/R þ SEW2871: 2.570.4, Po0.05) 24 h after ischemia. These changes were accompanied by 69% reduction in circulating lymphocytes, and 77 and 66% reduction in infiltrating neutrophils and macrophages in renal outer medulla, respectively (all Po0.01). The mRNA abundance of tumor necrotic factor-a (TNF-a), P-selectin, E-selectin, and intercellular adhesion molecule-1 (ICAM-1) was markedly increased by I/R injury (3.5-, 4.1-, 3.5-, and 4.8-folds, respectively, all Po0.05 vs sham). SEW2871 treatment partially reversed the upregulation of TNF-a, P-selectin, and ICAM-1 (47, 59, 54%, respectively, vs I/R control: 100%, all Po0.05). The reduction in protein expression of TNF-a, P-selectin, and ICAM-1 was further confirmed with immunofluorescence studies. These results suggest that SEW2871 ameliorates renal I/R injury by inhibiting lymphocyte egress and reducing pro-inflammatory molecules. This new class of renoprotective agent shows promise as a novel approach in preventing/treating ischemic acute renal failure.

Comparison of observed and simulated cement microstructure using spatial correlation functions

The microstructure of cement pastes, as revealed by SEM-BSE image analysis, was compared with a simulated structure generated by the University of Twente version of the CEMHYD3D hydration simulation model. The spatial array of unhydrated cement particles was simulated by the model. However, spatial features in capillary pore structure obtained by the simulation are different from the observed microstructure. This disagreement in the spatial structure is to be expected since there are
more &raquo; ... differences in porosity as represented by the two methods. Only coarse pores are detected in the SEM examination while the total capillary porosity and its whole spatial distribution are virtually simulated in the model. A subset of the visible pores must be different in spatial statistics from the universal set of total porosity. Care must therefore be taken in interpreting agreement between simulation output and microscopically observed microstructure in images.

Beyond the well-known role of proteolytic machinery in protein degradation and turnover, many specialized proteases play a key role in various regulatory processes. Thousands of highly specific proteolytic events are associated with normal and pathological conditions, including bacterial and viral infections. However, the information about individual proteolytic events is dispersed over multiple publications and is not easily available for large-scale analysis. CutDB is one of the first
more &raquo; ... ic efforts to build an easily accessible collection of documented proteolytic events for natural proteins in vivo or in vitro. A CutDB entry is defined by a unique combination of these three attributes: protease, protein substrate and cleavage site. Currently, CutDB integrates 3070 proteolytic events for 470 different proteases captured from public archives (such as MEROPS and HPRD) and publications. CutDB supports various types of data searches and displays, including clickable network diagrams. Most importantly, CutDB is a community annotation resource based on a Wikipedia approach, providing a convenient user interface to input new data online. A recent contribution of 568 proteolytic events by several experts in the field of matrix metallopeptidases suggests that this approach will significantly accelerate the development of CutDB content. CutDB is publicly available at http://cutdb.burnham.org.

Purpose Kampo (Japanese traditional herbal medicine) plays a certain role in the current health practices in Japan. Similar to other medical interventions, health-economic aspects of Kampo medicines should be assessed for the rational use of scarce healthcare resources. The purpose of this study is to review the published pharmacoeconomic evaluations on Kampo medicines in Japan, delineating the current situation in this area. Methods We searched the database "Ichushi (Japana Centra Revuo
more &raquo; ... a) Web (Ver.5)" which is the largest database of medical literature in Japan, using search terms of "Kampo" and "keizai" (economics). Literature published between 1983 and 2011 were eligible for searching. After primary screening, we selected articles regarded as full economic evaluations or cost studies. Structured abstracts designed especially for health-economic analyses of Kampo medicines were composed for them. Results Although we found 110 articles via initial electric search, only 11 articles on Kampo medicines (10 full economic evaluations and 1 cost analysis) were eligible for next phase, i.e., composing a structured abstract. The most common design of full economic evaluations is costconsequence analysis (CCA) (n=8), which did not aggregate costs and consequences in the conclusions. However, considering the measurement of consequences adopted in those CCAs, 6 evaluations were similar to cost-effectiveness analysis (CEA) and the other 2 were similar to cost-minimization analysis (CMA). No costutility analyses (CUA) were found in our review. Conclusion The published pharmacoeconomic evaluations on Kampo medicines in Japan are few and the quality of them needs to be improved. The spread of essential pharmacoeconomic knowledge among researchers is considered to be important for the future economic evaluations on Kampo medicines and its use in health policy and practices in Japan. Our research investigates approaches to building an augmented whiteboard in the context of continuous, long term office use. In particular, we pursued three avenues of research based on input from user studies: techniques for the management of space on the board, the ability to flexibly apply behaviors to support varied application semantics, and mechanisms for managing history on the board.
