The impact of thyroid hormones on lipid levels is primarily mediated through activation of promoter regions of the low-density lipoprotein (LDL) receptor, 3--hydroxy-3-methylglutaryl coenzyme A (HMG-CoA) reductase, cholesteryl-ester transfer protein (CETP) and hepatic lipase (HL) genes (2'5). Hypothyroidism is a well-established cause of secondary dyslipidaemia and the increased risk of atherosclerosis (6). It is characterized by hypercholesterolaemia and a marked increase of LDL. This is caused by a markedly decreased fractional clearance of LDL particles because of a reduced number of LDL receptors in the liver. However, conflicting results were obtained in different studies Jugoslov. Med. Biohem. 2002.; 21 (4) 331 Summary: Hypothyroidism is associated with dislipidaemia and an increased risk of atherosclerosis. The aim of this study was to examine the effect of disease and long-term levothyroxine replacement therapy on serum lipids in hypothyroid children. We measured concentrations of total cholesterol, HDL-cholesterol, LDL-cholesterol, triglycerides and thyroid-stimulating hormone in serum samples of 58 children with hypothyroidism (before and after therapy) and in 100 healthy controls. LDL-C and TC values were most markedly affected by the hypothyroid state. The values in the patients were on the average of 100% (LDL-C) and 54% (TC) higher than in the healthy controls. This finding is accordance with the known fact that LDL clearance is slower in hypothyroidism. TG concentrations were slightly higher (p<0.05) and HDL-C lower (p<0.02), but only in some age groups of patients. The effect of long-term L-T 4 replacement on LDL-C and TC was quantitatively most pronounced. The values obtained in patients after therapy were markedly lower than before therapy and did not differ significantly from the values found in the corresponding age groups of healthy children. Significantly lower concentrations of HDL-C and TG after L-T 4 administration were found only in children aged from 10 to 15 years. When we compared the lipid-lowering effect of L-T 4 replacement therapy in normometabolic versus hypometabolic patients, we obtained a significantly greater effect on TC and LDL-C values in the normometabolic than in hypometabolic group (p<0.01). TC and LDL-C concentrations obtained in the L-T 4 treated patients correlated well with the corresponding TSH levels (r = 0.770 and 0.725, respectively).

Za utvr|ivanje uticaja starosti deteta na ispitivani parametar isti su obra|eni u uzorku zdrave dece. Grupa zdrave dece N=100 uzrasta od 1 meseca do 18 godina podeljena je u 5 starosnih podgrupa. Posebnu grupu sa~injava uzorak uzet iz pupane vrpce, zdravo ro|ene dece. Grupa obolele dece je podeljena po istim starosnim grupama kao i zdrava grupa i to: u momentu otkrivanja bolesti (N=58) i u momentu kasnijeg pregleda, posle dugovremene supstitucione terapije sa L-T4 (N=56). Metaboli~ko stanje deteta utvr|eno je po kriterijumu specifi~nih kli-ni~kih znakova i simptoma. STABILITY STUDY OF AMLODIPINE BESYLATE AND BISOPROLOL FUMARATE IN AQUEOUS SOLUTIONS

Valuable information concerning stability of compounds can be obtained by using different media (water, hydrochloric acid or sodium hydroxide) for dissolution of active pharmaceutical substances. more » ... ical kinetics approach was used, and constant rate (k), half-time (t1/2) and activation energy (Ea) were used for prediction of compound stability degree. The stability of amlodipine besylate and bisoprolol fumarate were tested, both separately and in mixture, in water and in 0.01M HCl. All the investigated solutions were treated at two temperatures 25° and 70°C at the following time intervals: 0, 1 h, 24 h, 48 h and 72 h. Hydrophilic Interaction Liquid Chromatography – HILIC method, previously developed and validated, was used. On the basis of obtained results it was concluded that amlodipine-besylate was more stable in water than in acid medium, more stable in mixture rather than individually and more stable at lower temperatures. This was confirmed by the obtained values of monitored parameters: amlodipine besylate Ea = 30.68 kJ mol-1, k (25 °C) = 0.000333 mM h-1, k (70 °C) = 0.00169 mM h-1; amlodipine besylate in mixture Ea = 42,414 kJ mol-1, k (25 °C) = 1.27∙10-4 mM h-1, k (70 °C) = 0.0012 mM h-1. Based on the obtained approximate Ea value for bisoprolol fumarate in acid (59 kJ mol-1) and in water (56 kJ mol-1), bisoprolol fumarate showed excellent stability against the media in which it was studied. On the other hand, the temperature had a significant effect on the stability of bisoprolol fumarate. These results provide the relevant information about the stability of the tested active substances, and may be of importance during the development of an appropriate pharmaceutical product. A bigger influence on the stability of bisoprolol fumarate had a temperature effect.

Double-hit primary unilateral adrenal lymphoma with good outcome

Introduction. Primary adrenal non-Hodgkin's lymphoma (NHL) is a rare neoplasm with poor prognosis. On the other side, double-hit lymphomas with BCL2 and MYC translocation are characterized by advanced disease stage, extranodal and central nervous system involvements at presentation or disease progression. Case report. We reported a 73-year-old male patient with double-hit primary adrenal lymphoma and preserved adrenal function, showing a favorable clinical course. Computed tomography of abdomen
more » ... showed a 9 7 cm mass of the left adrenal gland. Laparatomy with left adrenalectomy was done and histological examination revealed diagnosis of a diffuse large B-cell NHL (DLBCL), non-GCB subtype. The patient was treated with 6 cycles of R-CHOP chemotherapy with reduced doses of doxorubicin because of the decreased left verticle ejection fraction. The patient was followed up regularly for 20 months with no evidence of tumor recurrence despite the inherently poor prognostic profile and double-hit phenotype of the disease. Conclusion. R-CHOP chemotherapy in combination with adrenalectomy can be an effective first-line regimen for primary adrenal DLBCL, despite the inherently poor prognostic profile (non-GCB subtype, bulky disease, elevated lactate dehydrogenase and double-hit phenotype of the disease). Uvod. Primarni adrenalni neho kinski limfom izuzetno je redak oblik limfoma i odlikuje se lošom prognozom. Sa druge strane, double-hit limfomi sa BCL2 i MYC translokacijom karakterišu se uznapredovalom boleš u u vreme postavljanja dijagnoze, esto prisutnom ekstranodalnom lokalizacijom bolesti i lošom prognozom. Prikaz bolesnika. Prikazali smo bolesnika starog 73 godine sa double-hit primarnim limfomom nadbubrega sa o uvanom adrenalnom funkcijom i povoljnim klini kim tokom. Kompjuterizovanom tomografijom abdomena registrovana je tumorska promena leve adrenalne žlezde, veli ine 9 7 cm, dok nije utvr eno prisustvo bolesti na drugim lokalizacijama. U injeno je kompletno hirurško ukljanjanje tumorske promene i na osnovu histološkog pregleda postavljena je dijagnoza difuznog B krupno elijskog limfoma, (DLBCL) non-GCB podtipa. Nakon hirurškog le enja primenjen je R-CHOP protokol sa redukovanim dozama adriablastina zbog smanjene ejekcione frakcije leve komore. I pored veoma lošeg prognosti kog profila bolesti (non-GCB tip, bulky bolesti, visokih vrednosti laktat dehidrogenaze, visokog komorbiditetnog skora i double-hit fenotipa) bolesnik je ve 20 meseci u kompletnoj remisiji. Conclusion. Imunohemioterapija (R-CHOP) u kombinaciji sa hirurškim le enjem je efikasna prva linija terapije kod bolesnika sa primarnim adrenalnim difuznim B-krupno elijskim limfomom i pored nepovoljnog prognosti kog profila i double-hit fenotipa bolesti. The aim of this study was to determine the content of magnesium in magnesium-chloride and magnesium-sulfate using the gravimetric analysis method, and to do the comparison of experimental results with calculations, stoichiometric calculation. Magnesium is one of the least present minerals in the soil and that is why, even in a well-balanced diet, a lack of magnesium is present. It is of great importance for the human body and is increasingly consumed using different nutritional supplements.

Determination of Vitamin C Content in Dietary Supplements

This research focused on determination of vitamin C content in pharmaceutical preparations using the titrimetic method of analysis. The content of vitamin C (ascorbic acid) was measured in twelve preparations. Pharmaceutical preparations that were used for the analysis were supplied in retail, on the territory of Republic of Srpska. In the iodometry, iodine is reduced by ascorbic acid and when it all is reacted, iodine that is in excess forms a blue-black complex with starch indicator, which marks the ending point of the titration.Based on the results obtained during the experimental work, it is evident that the percentage content of vitamin C in the compositions is in the interval from 95.24 to 99.64 %. Deviations from the prescribed values are not larger than 5 %. This analysis concluded that the highest percentage of vitamin C include samples of DM (99.64 %), Biofar's (99.64 %) and Pliva sample tablets with sustained release (99.50 %). The lowest percentage was determined in a sample Eunova-Multi-Vitalstoffe (95.24 %).

Late auditory ERP changes after sensorimotor rhythm neurofeedback training

Background/Aim. This pilot study aimed to investigate the effect of lower-beta or sensorimotor rhythm (SMR) (12-15 Hz) neurofeedback (NFB) training on amplitudes and latencies of late auditory event-related potentials (aERP) components N100, N200, P300 in Go-No go task of auditory discrimination. Methods. Each of 9 healthy participants aged 25-40 years (4 male) had 20 daily sessions of SMR neurofeedback training. The aERP was recorded 5 times: before NFB, after 5, 10, and 20 sessions, and one
more » ... nth after the last session. Results. The results showed a statistically significant decrease in N100, N200, and P300 latencies at Fz, Cz, and Pz regions. No significant effect of NFB training on amplitude was found. Conclusion. The obtained results suggest that the effects of NFB exerts its effect on the processes of auditory cognition. Cilj ove pilot studije je da ispita efekat Neurofidbek (NFB) treninga senzomotornog ritma (SMR) tj. niskog opsega beta ritma (12-15 Hz) na amplitude i latence komponenti kasnih auditivnih evociranih potencijala (AEP) N100, N200 i P300 tokom zadatka auditivne diskriminacije. Metod. Svaki od 9 zdravih ispitanika uzrasta od 25 do 40 godina (4 muškog pola) imao je 20 SMR NFB treninga (po jedan svakog dana). AEP su snimani 5 puta: pre primene NFB treninga, posle 5, 10, i 20 treninga i jedan mesec nakon poslednjeg treninga. Rezultati. Rezultati su pokazali statistički značajno smanjenje latenci N100, N200 i P300 komponenti u Fz, Cz i Pz regijama. Nije utvrđen statistički značajan efekat NFB treninga na amplitude komponenti N100, N200 i P300. Zaključak. Dobijeni podaci ukazuju na potencijalni efekat NFB treninga na poboljšanje kognitivnog procesa auditivne diskriminacije. 4 Ključne reči: auditivni evocirani potencijali; amplituda; latenca; N100; N200; P300; neurofidbek; senzomotorni ritam.

Opstruktivna apneja u snu (OSA) je hronično progresivno oboljenje sa visokom prevalencom u populaciji koje, bez pravovremene dijagnoze i terapije, može dovesti do značajnih posledica po kvalitet života pacijenata. OSA je čest komorbiditet kod pacijenata sa metaboličkim sindromom (MS) i kardiovaskularnim bolestima (KVB) i predstavlja važan faktor rizika za nastanak ovih oboljenja, a prisustvo nelečenog, teškog oblika OSA povezano je sa porastom ukupnog i mortaliteta usled koronarnih događaja.
more » ... jne studije su ukazale na vezu između MS i OSA, te je ovaj fenomen opisan kao poseban poremećaj -sindrom Z. Istraživanje uzročno-posledične veze između OSA i KVB je u velikoj meri otežano kompleksnom prirodom samog oboljenja. Smatra se da je kardiometabolički rizik u OSA udružen sa arterijskom hipertenzijom, insulinskom rezistencijom, endotelnom disfunkcijom, inflamacijom, dislipidemijom i oksidativnim stresom. Lečenje OSA se danas najefikasnije sprovodi neinvazivnom ventilacijom, pomoću uređaja koji obezbeđuje pozitivan pritisak u gornjim disajnim putevima (eng. continuous positive airway pressure, CPAP) i na taj način sprečava pojavu apneja tokom spavanja. Rezultati kliničkih studija su pokazali da CPAP terapija značajno poboljšava hemodinamske parametre, reguliše hipertenziju, povećava osetljivost na insulin i koriguje dislipidemiju. Buduća istraživanja bi trebalo da rasvetle da li je apneja u snu faktor rizika za KVB per se ili je ta veza posledica šireg patofiziološkog procesa, čiji je deo i OSA. Summary Obstructive sleep apnea (OSA) is a chronic, progressive disorder with a high prevalence in the population. Without timely diagnosis and therapy OSA can significantly affect the quality of life of the patients. OSA is a common co-morbidity in patients with metabolic syndrome (MS) and cardiovascular disease (CVD) and is an important risk factor for their development. The presence of untreated, severe OSA is associated with an increase in total and cardiovascular mortality. Numerous studies have pointed to the relationship between MS and OSA, and this phenomenon was described as syndrome Z. Investigation of the causal relationship between OSA and CVD has been greatly confounded by the complex nature of the disease itself. Cardiometabolic risk in OSA is associated with arterial hypertension, insulin resistance, endothelial dysfunction, inflammation, dyslipidemia, and oxidative stress. The treatment of OSA is now most effectively performed by continuous positive airway pressure (CPAP), a type of non-invasive ventilation which prevents the onset of sleep apnea. The results of clinical studies have shown that CPAP therapy significantly improves haemodynamic parameters, regulates hypertension, increases insulin sensitivity, and corrects dyslipidemia. Future investigations should clarify whether sleep apnea is a risk factor for CVD per se or is a consequence of a broader pathophysiological process, of which OSA is part.

Dijabetes melitus tip 2 je hronično oboljenje sa visokom prevalencom, koje se karakteriše hiperglikemijom, ali i poremećajima u metabolizmu lipida i proteina. Pacijenti sa tipom 2 dijabetesa imaju visok rizik za razvoj kardiovaskularnih bolesti (KVB) i upravo se dislipidemija smatra ključnim uzročnikom ovog povećanog rizika. Hipertrigliceridemija, snižena koncentracija holesterola u lipoproteinskim česticama visoke gustine (HDL-h) i promena u raspodeli lipoproteinskih čestica niske gustine
more » ... u smeru većeg udela malih čestica, bogatih trigliceridima, predstavljaju najvažnije promene lipidnog profila koje se odnose na dislipidemiju u dijabetesu. Kako je dijabetes stanje kontinuirane blage inflamacije niskog stepena i stalne produkcije slobodnih radikala, HDL lipoproteinske čestice takođe podležu strukturnim i funkcionalnim promenama, usled čega gube svoje ateroprotektivne osobine. Terapija dislipidemije kod pacijenata mlađih od 40 godina bez prisutnih drugih faktora rizika za razvoj KVB počinje promenom životnog stila, a kod pacijenata starijih od 40 godina lekovi izbora u terapiji su statini. Glikemijska i lipidna kontrola kod pacijenata sa tipom 2 dijabetesa značajno umanjuje rizik od nastanka KVB. Donepezil is a potent, selective, noncompetitive, and rapidly reversible inhibitor of acetylcholinesterase (AChEI) licensed for the treatment of Alzheimer disease (AD); and is the first and only AChEI licensed for the treatment of severe AD. Its efficacy as monotherapy, or in combination with the NMDA-agonist, memantine, has been documented in several randomised double-blind, placebo-controlled, short-term clinical trials, as well as long-term extension trials and observational studies.
more » ... l is a well tolerated drug that is generally safe as demonstrated even in patients with multiple co-morbidities receiving polypharmacy. It has been shown that donepezil improves cognition and global function in patients with mild-to-moderate AD; and long-term efficacy is maintained for up to 50 weeks. There is a dose-response relationship, with higher doses more likely to produce symptomatic benefit. Furthermore, donepeziltreated patients may improve cognitively and show global clinical improvement in all disease stages, including severe AD. Less consistent results in all disease stages were obtained on measures of function and behavior, and observations of mood. No effect on transition to AD has been found in long-term, randomized clinical trials in mild cognitive impairment (MCI). Cost-effectiveness of the treatment has been questioned by one long-term open-label societal study of 2-years duration. This study reported modest improvement of cognition but no statistically significant benefits during donepezil treatment as compared to placebo, in terms of rates of institutionalization and progression toward greater disability. However, there is a need for further research on clinically meaningful outcomes and treatment benefits favored by patients and caregivers, which are traditionally not defined as outcomes in clinical trials. Likewise, we need to know how to select responders, what is an optimal AChE inhibition particularly during the long-term treatment, in which patients the dosage should be increased for a sustained benefit, what is the optimal duration of treatment and when is meaningful to stop the treatment. After almost two decades of donepezil use in everyday clinical practice these issues are still unresolved.

The intensive industrial and technological development results in an appearance of a large amount of pollutants and toxic substances in the biosphere including large quantities of heavy metals. Cadmium accumulates in the human body with a biological half-life from 10 to 30 years and could cause very serious toxic effects. Cosmetic products are potentially a source of cadmium as well as some other heavy metals mostly due to the frequent and uncontrolled usage of cosmetics of poor аnd/or suspicious quality. The goal of the study is to determine the cadmium content in various cosmetic products (lipsticks, eyeshadows and blushes) present in the markets of the Republic of Srpska. The content of Cd was determined with atomic absorption spectrophotometry (AAS) after the extraction with diluted HCl and aqua regia. The results of the research show that the cadmium concentrations in the investigated products are in accordance with the regulations of the Republic of Srpska (Regulations on safety of consumer goods; "Official Gazette RS" – No.17/15).

Voltammetric determination of lead and copper in wine by modified glassy carbon electrode

This paper describes the determination of Pb and Cu with a Nafion-modified glassy carbon electrode and MnCo2O4 nanoparticles as working electrode for anodic stripping voltammetry. Pb and Cu were accumulated in HCl/KCl (0.1 mol dm -3 ) at a potential of -1.4 V (vs. Ag/AgCl electrode) for 480 s, followed by a linear sweep anodic stripping voltammetry (ASV) scan from -1.0 to +0.5 V. more » ... Cu, respectively. Effect of sample dilution, accumulation time and potential were optimized. A study of interfering substances was performed. A significant increase in current was obtained at the modified electrode in comparison with the bare glassy carbon electrode. The modified electrode was successfully applied for determination of Pb and Cu in wine samples after a simple preparation procedure. Pb and Cu content in wine was used for estimation of the target hazard quotient (THQ) values for minimal and maximal levels of the metals.
