Proposing a manuscript peer-review checklist

-In the Internet-able era reviewers are faced with an increased number of manuscripts and decreased time to review. In order to maintain the same, if not higher level of quality in the peer-review process, a net gain in productivity is required. Our goal is to present a manuscript peer-review checklist to help reviewers achieve this secondary yet critical task in a more systematic fashion. Methods -To this end, we have compiled, structured and processed information from nine reference
more &raquo; ... guidelines or directives, resulting in a 70 criterias checklist. We ensured that criterias were assessable based on the verification, validation and evaluation paradigm. Results - The checklist is presented in the manuscript, along with a description of a review workflow. Findings It is hoped that the checklist will be widely disseminated, and we are looking for feedback on validation and improvements in order to perform a quantitative study on productivity gains using this tool.

- Decision scores and ethically mindful algorithms are being established to adjudicate mechanical ventilation in the context of potential resources shortage due to the current onslaught of COVID-19 cases. There is a need for a reproducible and objective method to provide quantitative information for those scores. Purpose - Towards this goal, we present a retrospective study testing the ability of a deep learning algorithm at extracting features from chest x-rays (CXR) to track and predict
more &raquo; ... ogical evolution. Materials and Methods - We trained a repurposed deep learning algorithm on the CheXnet open dataset (224,316 chest X-ray images of 65,240 unique patients) to extract features that mapped to radiological labels. We collected CXRs of COVID-19-positive patients from two open-source datasets (last accessed on April 9, 2020)(Italian Society for Medical and Interventional Radiology and MILA). Data collected form 60 pairs of sequential CXRs from 40 COVID patients (mean age +/- standard deviation: 56 +/- 13 years; 23 men, 10 women, seven not reported) and were categorized in three categories: Worse, Stable, or Improved on the basis of radiological evolution ascertained from images and reports. Receiver operating characteristic analyses, Mann-Whitney tests were performed. Results - On patients from the CheXnet dataset, the area under ROC curves ranged from 0.71 to 0.93 for seven imaging features and one diagnosis. Deep learning features between Worse and Improved outcome categories were significantly different for three radiological signs and one diagnostic (Consolidation, Lung Lesion, Pleural Effusion and Pneumonia; all P < 0.05). Features from the first CXR of each pair could correctly predict the outcome category between Worse and Improved cases with 82.7% accuracy. Conclusion - CXR deep learning features show promise for classifying the disease trajectory. Once validated in studies incorporating clinical data and with larger sample sizes, this information may be considered to inform triage decisions.

Label fusion is used in medical image segmentation to combine several different labels of the same entity into a single discrete label, potentially more accurate, with respect to the exact, sought segmentation, than the best input element. Using simulated data, we compared three existing label fusion techniques—STAPLE, Voting, and Shape-Based Averaging (SBA)—and observed that none could be considered superior depending on the dissimilarity between the input elements. We thus developed an
more &raquo; ... al, hybrid technique called SVS, which selects the most appropriate technique to apply based on this dissimilarity. We evaluated the label fusion strategies on two- and three-dimensional simulated data and showed that SVS is superior to any of the three existing methods examined. On real data, we used SVS to perform fusions of 10 segmentations of the hippocampus and amygdala in 78 subjects from the ICBM dataset. SVS selected SBA in almost all cases, which was the most appropriate method overall.

Capturing early changes in the brain related to Alzheimer's disease may lead to models that successfully predict cognitive decline and the eventual onset of dementia, well ahead of onset of clinical symptoms. In this study we used both hippocampal volume and our hippocampal driven SNIPE score to show which marker better captures Alzheimer's related changes in a large dataset of normal controls (N=515) from the ADNI study, comparing controls that remain cognitively stable and controls that
more &raquo; ... ss to either MCI or Alzheimer's dementia during 10 years of follow-up (median follow-up: 30 months). We measured hippocampal volume and SNIPE score and found that the effect size to differentiate between stable and progressor groups was significantly larger for SNIPE score than for volume. Our results also show that there is a significant age-related difference between groups for both markers, and the difference is greater with the SNIPE score. Our experiments show that considering high sensitivity of our SNIPE score regarding to early AD-related brain changes, this marker is a better candidate in comparison to hippocampal volume for predicting the future onset of dementia.

Temporal Lobe Epilepsy Surgical Outcome Prediction [chapter]

We wished to study pre-operative T1-weighted MRI of intractable temporal lobe epilepsy (TLE) patients who had undergone selective amygdala-hippocampectomy as part of their surgical treatment. We performed a voxel-based morphometry study of gray and white matter (GM,WM) concentration changes by comparing TLE patients with positive and negative surgical outcome. GM concentration changes were primarily located in the left lateral temporal neocortical region, while more extensive changes were found

Beware of White Matter Hyperintensities Causing Systematic Errors in Grey Matter Segmentations! [article]

Volumetric estimates of subcortical and cortical structures, extracted from T1-weighted MRIs, are widely used in many clinical and research applications. Here, we investigate the impact of the presence of white matter hyperintensities (WMHs) on FreeSurfer grey matter (GM) structure volumes and its possible bias on functional relationships. Methods: T1-weighted images from 1077 participants (4321 timepoints) from the Alzheimer's Disease Neuroimaging Initiative were processed with FreeSurfer
more &raquo; ... on 6.0.0. WMHs were segmented using a previously validated algorithm on either T2-weighted or Fluid-attenuated inversion recovery (FLAIR) images. Mixed effects models were used to assess the relationships between overlapping WMHs and GM structure volumes and overal WMH burden, as well as to investigate whether such overlaps impact associations with age, diagnosis, and cognitive performance. Results: Participants with higher WMH volumes had higher overalps with GM volumes of bilateral caudate, cerebral cortex, putamen, thalamus, pallidum, and accumbens areas (P < 0.0001). When not corrected for WMHs, caudate volumes increased with age (P < 0.0001) and were not different between cognitively healthy individuals and age-matched probable Alzheimers disease patients. After correcting for WMHs, caudate volumes decreased with age (P < 0.0001), and Alzheimers disease patients had lower caudate volumes than cognitively healthy individuals (P < 0.01). Uncorrected caudate volume was not associated with ADAS13 scores, whereas corrected lower caudate volumes were significantly associated with poorer cognitive performance (P < 0.0001). Conclusions: Presence of WMHs leads to systematic inaccuracies in GM segmentations, particularly for the caudate, which can also change clinical associations. While specifically measured for the Freesurfer toolkit, this problem likely affects other algorithms.
