We report the case of a 54-year-old woman with asthma and atopic dermatitis who presented a white spot on the genitalia. Histologic examination showed numerous eosinophils in the epithelium and the dermis. Eosinophilic esophagitis is defined as an esophageal disease characterized clinically by symptoms related to esophageal dysfunction and histologically by an eosinophil-predominant inflammation. We discuss the possible relationship between the two diseases.

Effect of Oral Valaciclovir on Herpetic Keratitis

Purpose: To examine the efficacy of valaciclovir (VACV) oral formulation as an alternative to topical treatments in a case of herpetic keratitis. Methods: The patient was a 61-year-old man who presented with dendritic keratitis in his left eye. After recognizing his difficulty in using eye ointment, we prescribed oral VACV 500 mg tablets twice daily for 7 days. more » ... culated with herpes simplex virus type 1 (HSV)-1 in each eye and treated with oral VACV 50 or 100 mg/kg twice daily, oral acyclovir (ACV) 50 mg/kg 5 times/d, 3% ACV eye ointment (ACV-O) 5 times/d, 3% ACV eye drops 5 times/d, or control for 5 days. Results: After 7 days, the patient's lesion was observed healed. Corrected left visual acuity was also improved, and HSV DNA was below detectable level. In the mouse study, slit-lamp examination on days 3, 4, 5, and 7 revealed that all 5 ACV and VACV treatment groups were significantly more effective in improving symptoms of herpetic epithelial keratitis versus control (P , 0.05). Moreover, VACV 100 mg/kg was superior to other treatments. Viral titers in mouse eyeball and trigeminal ganglia were lowest in the VACV 100 mg/kg group versus other treatment groups. Conclusion: Our case example suggests that when frequent application, blurred vision, and foreign body sensation after ACV-O application cause difficulty for patients to follow treatment, oral VACV might be an effective and safe option for patients with herpetic keratitis.

Acute epidural hematoma due to spinal venous angioma: A case report

A B S T R A C T Background: Spinal epidural cavernous angioma was reported about 4% of all spinal epidural tumors, mostly generating as a primary lesion in the vertebral bone. Spinal epidural hematoma (SEDH) due to angioma without primary origin was very rare, which reported in the English literatures was only 10 cases including our case. The main symptoms were numbness and pains and motor weakness owing to the acute compression signs of cord and roots. Emergency surgical treatment was required
more » ... for the good surgical results. We present a rare case of cervical acute epidural hematoma due to ruptured venous angioma. Case presentation: A 78-year-old man, who suffered from sudden severe right suboccipitalgia., and sent to our hospital. He also noticed progressive numbness and motor weakness over his bilateral upper and lower extremities after the onset. Cervical CT and MRI showed acute intracanal hematoma over the right side of the cervical spine at C2 to C6. Emergency operation was performed 13 h after the onset. His sensory and motor symptoms were disappeared quickly after the operation. Pathological examination revealed spinal epidural hematoma due to venous angioma. Conclusion: This case presented with acute progressive deterioration of paresis and sensory disturbance, and required emergency operation. Early diagnosis and emergency surgical treatment are essential for the therapy of acute spinal epidural hematoma due to venous angioma.

more » ... ration of the supraclinoid segment of the internal carotid artery (ICA) at the origin of the posterior communicating artery (PCoA). The patient had a fusiform aneurysm at the proximal branch of the PCoA, which was successfully clipped, sparing the anterograde blood flow. The double origin and fenestration of the PCoA branching off at the C 2 segment of the left ICA suggested that this anomalous fenestration might have developed as the origin of the PCoA rather than the supraclinoid ICA during the early embryonal stage.

Small RNA attracts increasing interest based on the discovery of RNA silencing and the rapid progress of our understanding of these phenomena. Although recent studies suggest the possible existence of yet undiscovered types of small RNAs in higher organisms, many studies to profile small RNA have focused on miRNA and/or siRNA rather than on the exploration of additional classes of RNAs. Results: Here, we explored human small RNAs by unbiased sequencing of RNAs with sizes of 19-40 nt. We provide
more » ... substantial evidences for the existence of independent classes of small RNAs. Our data shows that well-characterized non-coding RNA, such as tRNA, snoRNA, and snRNA are cleaved at sites specific to the class of ncRNA. In particular, tRNA cleavage is regulated depending on tRNA type and tissue expression. We also found small RNAs mapped to genomic regions that are transcribed in both directions by bidirectional promoters, indicating that the small RNAs are a product of dsRNA formation and their subsequent cleavage. Their partial similarity with ribosomal RNAs (rRNAs) suggests unrevealed functions of ribosomal DNA or interstitial rRNA. Further examination revealed six novel miRNAs. Conclusion: Our results underscore the complexity of the small RNA world and the biogenesis of small RNAs.
