Gossypiboma refers to incidences of retained foreign bodies usually forgotten within the abdominal cavity at the end of an operation. It may be a surgical sponge, gauze pad, or other forms of textiles, instruments. Aims and objective: To try and identify the factors which are responsible for the formation of Gossypiboma. Material and method: A Retrospective study was carried out in patients presented with postop (following open surgery) complications; diagnosis is made first followed by looking
more &raquo; ... into possible causative factors. Results: In our study of 900 patients (age group>20 yrs; underwent open surgery in past); Out of 900 patients included in the study, it was found that 5 patients were found to have retained foreign objects in the abdomen. Among four patients two were operated for caesarian-section, one for open cholecystectomy, and one underwent laparotomy for intestinal perforation. The Surgical safety checklist was not being followed in all five cases; the adequacy and compatibility of OT staff was questionable in 2 cases. Conclusion: By following standard surgical protocol of Surgical safety checklist along with proper teamwork (good rapport and communication among the dedicated OT staffs and operating surgeon); allocation of ample working hours; regular teaching and training of OT staff personnel about the maintenance of standard surgical protocols and finally addressing the consequences of legal repercussions as well as ignominy faced by hospital administration; such rarely occurring incidences can be minimized to never happening incidence. Distributional reinforcement learning (DRL) is a recent reinforcement learning framework whose success has been supported by various empirical studies. It relies on the key idea of replacing the expected return with the return distribution, which captures the intrinsic randomness of the long term rewards. Most of the existing literature on DRL focuses on problems with discrete action space and value based methods. In this work, motivated by applications in robotics with continuous action space
more &raquo; ... ontrol settings, we propose sample-based distributional policy gradient (SDPG) algorithm. It models the return distribution using samples via a reparameterization technique widely used in generative modeling and inference. We compare SDPG with the state-of-art policy gradient method in DRL, distributed distributional deterministic policy gradients (D4PG), which has demonstrated state-of-art performance. We apply SDPG and D4PG to multiple OpenAI Gym environments and observe that our algorithm shows better sample efficiency as well as higher reward for most tasks.

According to Ancient Indian philospers all matter of this universe is made up of five basic constituents commonly called panch tatavas. Indian philospher Maharishi Kanad around 500 BC postulated that matter is divisible. The smallest indivisible matter were named as parmanu. Greek philosphers named this indivisible particles atoms. In 1808 John Dalton suggest that the atom was indivisible and industructible. After the discovery of electron and proton inside the atom, led to the failure of
more &raquo; ... s theory. In the thirty years between the discovery of neutron in 1932 to the middle of 1962, more than thirty new particles were found, which were thought to be the builders of matter in the universe. Therefore it was established beyond doubt that all the particles discovered are not fundamental but have complex nature. In 1964 Gell-Mann and Zweing proposed that most of these particles called Quarks. Nearly 200 elementary particles were discovered so far. It appears that our search for the ultimate builder is not in a concluding stage but we are reaching towards the goal in stages. In this way we can say that the mysterious world of elementary particles and Quark is still mysterious. The same is written in Vedas and Purana that the search of ultimate particle will go on and on.

For testing goodness of fit, we consider a class of U-statistics of overlapping spacings of order two, and investigate their asymptotic properties. The standard U-statistic theory is not directly applicable here as the overlapping spacings form a dependent random sequence. The asymptotic distribution of the statistics under the null hypothesis and under a sequence of local alternatives are derived. In terms of the Pitman ARE, the U-statistic based on Gini's mean square difference of overlapping

Cloud computing platforms are becoming increasingly popular for e-commerce applications that can be scaled on-demand in a very cost effective way. Dynamic provisioning is used to autonomously add capacity in multi-tier cloud-based applications that see workload increases. While many solutions exist to provision tiers with little or no state in applications, the database tier remains problematic for dynamic provisioning due to the need to replicate its large disk state. In this paper, we explore
more &raquo; ... virtual machine (VM) cloning techniques to spawn database replicas and address the challenges of provisioning shared-nothing replicated databases in the cloud. We argue that being able to determine state replication time is crucial for provisioning databases and show that VM cloning provides this property. We propose Dolly, a database provisioning system based on VM cloning and cost models to adapt the provisioning policy to the cloud infrastructure specifics and application requirements. We present an implementation of Dolly in a commercial-grade replication middleware and evaluate database provisioning strategies for a TPC-W workload on a private cloud and on Amazon EC2. By being aware of VM-based state replication cost, Dolly can solve the challenge of automated provisioning for replicated databases on cloud platforms.

We study the intersection lattice of the arrangement A^G of subspaces fixed by subgroups of a finite linear group G. When G is a reflection group, this arrangement is precisely the hyperplane reflection arrangement of G. We generalize the notion of finite reflection groups. We say that a group G is generated (resp. strictly generated) in codimension k if it is generated by its elements that fix point-wise a subspace of codimension at most k (resp. precisely k). We prove that the alternating subgroup Alt(W) of a reflection group W is strictly generated in codimension two, moreover, the subspace arrangement of Alt(W) is the truncation at rank two of the reflection arrangement A^W. Further, we compute the intersection lattice of all finite subgroups of GL_3(R), and moreover, we emphasize the groups that are "minimally generated in real codimension two", i.e, groups that are strictly generated in codimension two but have no real reflection representations. We also provide several examples of groups generated in higher codimension.
