Gout with hearing loss

Gout is regarded as an autoinflammatory disease induced by the deposition of monosodium urate crystals in the synovial fluid and periarticular tissues. It is characterized by joint symptoms and uric acid nephropathy, as well as other organs involved due to inflammation. Here, we presented a rare case of gout with hearing loss. A 26-year-old Chinese man presented to the Otolaryngology Department with a 1-year history of bilateral ears hearing loss. He had no history of ear injury, fever, rash,
more » ... adache, nasitis, red eyes, hypertension, and diabetes, and denied exposure to loud noise or ototoxic drugs. Otoscopic examination showed that eardrums were normal. Enhanced magnetic resonance imaging (MRI) scanning detected no abnormalities of inner acoustic meatus or intracranial space-occupying lesion. Auditory brainstem responses (ABR) indicated that the right threshold was increased to 50, and the latency of left V wave and left I-V waves interval were slightly prolonged. Pure tone audiometry (PTA) demonstrated elevated thresholds of 35 dB at 8 kHz in the left ear, while 75 dB and 55 dB at 4 kHz and 8 kHz in the right ear, respectively. Testing for distortion product otoacoustic emissions (DPOAE) found abnormal otoacoustic emissions in the 6-8 kHz frequency of the left ear, and in the 4 kHz and 6-8 kHz frequency of the right ear. He was diagnosed with nerve deafness by the otolaryngologist. Meanwhile, we learned that he had a 3-year history of gout. The right first metatarsophalangeal joint was hurt once; the symptom alleviated voluntarily. Annual follow up showed that serum uric acid increased but without any treatment. Physical examination showed no joint swelling and tenderness, and no ectopic tophus. Laboratory findings showed an increased serum uric acid of 730 mmol/L. Complete blood count, liver and kidney function, C-reactive protein, and erythrocyte sedimentation rate were normal, while anti-nuclear antibodies and antineutrophil cytoplasmic antibodies were negative, and renal ultrasound showed no abnormality. Based on the clinical manifestations and laboratory results, he was diagnosed as gout complicated with nerve deafness. After 3 months of allopurinol monotherapy (100 mg, thrice a day), he had no arthritis and the serum uric acid level was decreased to 362 mmol/L. Intriguingly, his symptom of hearing loss was obviously relieved. Meanwhile, the results of ABR followup was improved with normalization of waves, but PTA and DPOAE results showed no remarkable changes.

A Vertically Stacked Quasi-Elliptic Waveguide Filter with Crossly Coupling Vias

This paper proposes a four-pole quasi-elliptic waveguide filter using multilayer low-temperature cofired ceramic (LTCC) technology. The cavities of the filter can be stacked vertically, while a pair of through hole vias are used to realize the cross coupling path for the quasi-elliptic response. Thus, the circuit size can be reduced by the overlap of the cavity resonators. The proposed filter has 20% size reduction in comparison with a conventional four-pole quasi-elliptic waveguide filter.

One-step green synthesis of graphene nanomesh by fluid-based method [article]

A fluid-based method is demonstrated for preparing graphene nanomeshes (GNMs) directly from pristine graphite flakes by a one-step process. The high efficiency is attributed to the combination of fluid-assisted exfoliation and perforation of the graphene sheets. Atomic force microscopy shows that the as-produced GNMs are less than 1.5 nm thick. The total area of the pores within 1 um2 of the GNM sheet is estimated as ~0.15 um2 and the pore density as ~22 um-2, The yield of GNMs from pristine
more » ... phite powder and the power consumption for per gram GNM synthesis are evaluated as 5 wt% and 120 kWh, respectively. X-ray photoelectron spectroscopy, infrared spectroscopy, elemental analysis and Raman spectroscopy results indicate the purity of the GNMs and thus it is a green efficient method. The present work is expected to facilitate the production of GNMs in large scale.

Evaluation of Transplacental Antibody Transfer in SARS-CoV-2-Immunized Pregnant Women

Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection during pregnancy could result in adverse perinatal outcome. Clinical data on the assessment of the immune response in vaccinated pregnant women and subsequent transplacental antibody transfer are quite limited. Objective: To assess maternal and neonatal neutralizing antibody levels against both wildtype and Delta (B.1.617.2) variants after maternal mRNA vaccination. Study Design: This cohort study was conducted 29 pregnant
more » ... men who were vaccinated at least one dose of Moderna (mRNA-1273) vaccine. Both neutralizing antibody (wildtype and Delta variant) and S1 receptor binding domain IgG antibody levels were evaluated in maternal and cord blood on the day of delivery. Results: Superiority of antibody level was significant in fully vaccinated women compared with the one-dose group (maternal sera, median, 97.46%; cord sera, median, 97.37% versus maternal sera, median, 4.01%; cord sera, median, 1.44%). No difference in antibody level was noted in relation to interval of second immunization to delivery in the two-dose group (95.99% in 0–2 weeks, 97.45% in 2–4 weeks, 97.48% in 4–8 weeks, 97.72% in 8–10 weeks). The most pronounced reduction was observed for the Delta variant. The wildtype neutralizing antibody level of full-vaccinated women was not influenced by the pertussis vaccination. Conclusion: The data underscore the importance of full vaccination in pregnancy and support the recommendation of COVID-19 immunization for pregnant women. The lower level of vaccine-induced neutralizing antibodies for the Delta variant indicates insufficient protection for mother and newborn and highlights the need for development of effective vaccine strategies.
