The aim of this study was to evaluate glomerular C3c deposits of Henoch -Schonlein purpura nephritis (HSPN) in children. Fifty-one patients aged 7-15 years (20 males and 31 females) were studied. On histological investigation , crescent formation seen under light microscopy and subepi thelial electron dense deposits (EDD) under electron microscopy were found to be related to the degree of proteinuria and the duration of proteinuria and / or hematuria . more » ... s performed on C3c-positive patients (n=22) and C3c -negative patients (n=25). Histological findings, such as crescent formation and subepithelial EDD , had no relation to glomerular C3c-deposits. At renal biopsy, C3c deposits were positive in 65 % of patients with heavy proteinuria (>100mg/kg/day), and in 30 % of mild proteinuria patients (<50mg/kg/day) . The diff erence between the two groups was statistically significant (p<0 .05). The duration of proteinuria and/or hematuria in C3c-positive patients had a tendency to persist in comparison with that in C3c-negative cases. Renal biopsies on many cases of C3c-negarive patients were performed following the lapse of three months, while the biopsies on patients showing global (+) C3c deposits (n=15) were conducted within the three-month period . These results suggest that glomerular C3c deposits influence the clinical conditions of patients with HSPN , and complement activation is generated in the early stage of HSPN.

Fujita) (Backgraund) Conventional transurethral Nd: YAG laser ablation of the prostate does not significantly reduce the volume of prostate and patients frequently experience post-operative urinary retention. Thus in the present study, prostatic tissue was extensively vaporized with laser fiber to produce a definite cavity during the operation. (Methods) Between August 1993 and April 1995, laser prostatectomy utilizing UltralineTM was carried out on 91 patients with BPH, 38 of these
more » ... ients had histories of urinary retention. The size and form of the prostate were examined by transurethral ultrasonography (TUUS) prior to surgery and ablation was conducted based on the findings. The results were checked by TUUS a second time. (Results) Operating time was 84. 1 minutes on the average including the time for systematic needle biopsy and cystostomy for irrigation. Total irradiation dose was 132, 035J on the average. The catheter was removed at an average 5. 8 days postoperatively. I-PSS was noted to improve from 15. 3 to 5. 6, maximum flow rate, 9. 3 to 15. 8 ml/s, average flow rate, 4. 6 to 9. 2 ml/s and residual urine, 76. 0 to 15. 9 ml. The volume of the prostate was reduced by 31. 5% (39. 3 to 26. 6 ml) and the patients could urinate for more than a year without discomfort. No serious complications were encountered though epididymitis was noted in 5 and urethral stricture in 10 patients. (Conclusion) The present laser prostatectomy is less invasive than TUR-P and the results might be equal to it.

Surgical treatment for medically refractory myasthenic blepharoptosis

Purpose: Currently, only a few reports have recommended surgery as a suitable treatment for blepharoptosis associated with myasthenia gravis. The present study aims to introduce our surgical criteria, surgical options, outcomes, and precautions for medically refractory myasthenic blepharoptosis. Patients and methods: Eight patients who failed to respond to at least 2 years of medical treatment and who underwent blepharoptosis surgery, from January 2008 to December 2011, were enrolled in this
more » ... dy. Medical records, photographs, and questionnaire results regarding postoperative status were evaluated. Of the eleven procedures performed, four involved frontal suspension, four involved external levator advancement, one involved nonincisional transconjunctival levator advancement, and two involved subbrow blepharoplasty with orbicularis oculi muscle tucking. The margin reflex distance improved postoperatively in seven patients. Results: Seven patients had very minimal scarring, and one had minimal scarring. Five patients showed no eyelid asymmetry, one had subtle asymmetry, and two had obvious asymmetry. Seven patients were very satisfied, and one patient was satisfied with the overall result. Postoperative complications included mild lid lag with incomplete eyelid closure, prolonged scar redness, and worsened heterophoria. No patient experienced postoperative exposure keratitis or recurrent blepharoptosis during the study period. Conclusion: Our results indicate that blepharoptosis surgery is effective for patients with myasthenia gravis, especially those with residual blepharoptosis despite multiple sessions of medical treatments. We recommend that neurologists and surgeons collaborate more systematically and discuss comprehensive treatment plans to increase the quality of life for patients with myasthenia gravis.

Synthesis and herbicidal activities of .ALPHA.-isocyanocycloalkylideneacetamides

Various a-isocyanocycloalkylideneacetamides were synthesized by the reaction of isocyanoacetamides with ketones and following by dehydration. These compounds were examined for their inhibitory activity against the germination of rice, cucumber and radish seeds, and for their herbicidal effects on rice, tomato and weed seedlings. Amongthem, a-isocyanocyclohexylideneacetylpiperidine showed selective herbicidal activity against the broad-leaf plants.
