Floating thrombus in the proximal aortic arch

Floating thrombus in the aortic arch is an unusual source of systemic embolism. Surgical removal of the thrombus is a therapeutic option, because thrombolytic therapy carries the risk of partial lysis and repeat embolization. We report a case involving a large floating thrombus that originated in the proximal aortic arch and extended into the left subclavian artery and the descending aorta, and we discuss the surgical management.

Transvenous approach via inferior petrosal sinus is one of the most widely used route for endovascular treatment of dural arteriovenous fistula (dAVF) around cavernous sinus or carotid-cavernous fistula (CCF). When it is not possible, however, direct access through superior ophthalmic vein (SOV) could be considered alternatively. A 72-year-old female admitted to our hospital presented unilateral orbital pain, exophthalmos, and 6 th nerve palsy, which had been being aggravated during a month.
more » ... netic resonance imaging and conventional angiography revealed dAVF that originated from internal maxillary artery leaking into SOV. We attempted the conventional venous approach via femoral vein-external jugular vein and facial vein. However, we failed to proceed the microcatheter into the left SOV owing to its tortuous and stenotic point. We chose an alternative treatment strategy, surgical exposure of the SOV following linear incision on eyelid and direct cannulation. Two temporary clips (Yasargil mini curved) seized the SOV and FAST-Cath TM for children (4 Fr, 8.5 cm) was inserted. We used a smaller sheath, which is shorter and softer than conventional endovascular catheter. It was beneficial in SOV puncture and catheter anchoring.

Toxocariasis is a parasite infection of a human host caused by Toxocara canis. Most human infections occur in persons who eat contaminated food containing embryonated eggs or larvae. Toxocariasis can produce various symptoms associated with eosinophilic infiltration into various internal organs such as the liver, lungs, eyes, or central nervous system. more » ... ned of right upper abdominal pain. Several imaging studies revealed acute cholecystitis and pericarditis with pericardial effusion. Marked eosinophilia was observed in her peripheral blood and pericardial fluid. A high titer of antibodies against T. canis was discovered during the search for the cause of the eosinophilia. All clinical features and the eosinophilia improved dramatically after treatment with albendazole. We report a case of simultaneous onset of acute cholecystitis and pericarditis associated with marked eosinophilia caused by T. canis.  요 약:본 연구에서는 개구제형 트윈 거더 교량의 곡률에 따른 상부수평브레이싱 부재력 산정식의 적절성 및 적용성을 검토하였다. 이를 위해 3경간 연속교를 대상으로 곡률반경을 50m에서 1, 200m로 변화시켜 상부수평브레이싱의 부재력 산정식과 3차원 유한요소해석을 통한 부 재력을 비교하였다. 그리하여 상부수평브레이싱의 공칭부재력을 계산할 수 있는 보정계수를 제안하였다. 특히 상부수평브레이싱 수직재 부재력 산정식은 3차원 유한요소해석결과와 잘 부합하지 않는 것으로 나타났는데, 이는 가로보의 유무에 따른 교축직각방향 강성의 영향이라 판단된 다. 그리고 사재와 수직재의 휨, 비틀림, 뒤틀림 및 수평분력의 영향을 곡률별로 도식화하여 나타내었다. The bracing forces of top lateral bracings from the results of numerical equations are compared to those of 3-dimensional finite element analyses. New modifying factors applicable in computing the nominal member forces of top lateral bracings were suggested. The numerical equations were derived based on one girder system, and it is shown that the numerical equations exhibit some errors compared with 3D FEA results. The main reason for this phenomenon lies on the number of girders. The twin girder system has an external cross-beam between inner and outer girder. It also has larger lateral stiffness than the single girder system. Role of Image-Enhanced Endoscopy in Pancreatobiliary Diseases

Detection of tumors of the ampulla of Vater, including ampullary adenoma, has been improved by routine screening endoscopic procedures and imaging modalities. Endoscopic resection by endoscopic papillectomy is rapidly replacing classic surgical resection and is a less invasive procedure. Endoscopic resection can have a role not only in the final histopathologic diagnosis but also as a definite therapeutic option. However, the indications for endoscopic resection are not fully established, and

Significant complications, including severe pancreatitis, intractable bleeding and duodenal perforation, are rare but can occur, especially in less experienced hands. Severe pancreatitis is the most feared complication, but it can be prevented by pancreatic duct stent insertion in most cases. However, in some cases, pancreatic stenting can be challenging after resection. Incomplete resections are sometimes performed to avoid complications. Endoscopic surveillance is also important for identifying and managing remnant adenomatous tissue or recurrent lesions. Further technical development is needed to expand the indications for this procedure, minimize complications and ensure a high success rate. (Gut Liver 2014;8:598-604)

Paraganglioma in the posterior mediastinum: a case report

Background Paragangliomas are tumors that arise from extra-adrenal chromaffin cells. Herein, we present a rare case of a functional paraganglioma in the posterior mediastinum. Case presentation A 36-year-old man presented with paroxysms of chest pain and headache. At presentation, the patient's blood pressure was 190/120 mmHg. Chest computed tomography and magnetic resonance imaging revealed a left paravertebral mass in the posterior mediastinum. 123I-metaiodobenzylguanidine scanning revealed
more » ... cally high tracer uptake in the left paravertebral area. The metanephrine level in the urine was elevated, confirming a rare, catecholamine-producing, functional paraganglioma in the posterior mediastinum. Before surgery, the patient was prepared by orally administering α- and β-adrenergic blockers. The mass was then resected via a lateral thoracotomy. The metanephrine level in urine was normal 24 h after surgery. Conclusions Paragangliomas in the posterior mediastinum are very rare, but more than half of all cases are functional. The associated symptoms are curable with complete resection, and long-term follow-up for recurrence is important.

Support Vector Machine Based Bearing Fault Diagnosis for Induction Motors Using Vibration Signals

In this paper, we propose a new method for detecting bearing faults using vibration signals. The proposed method is based on support vector machines (SVMs), which treat the harmonics of fault-related frequencies from vibration signals as fault indices. Using SVMs, the cross-validations are used for a training process, and a two-stage classification process is used for detecting bearing faults and their status. Effect of Cation Influx on the Viability of Freeze-Dried Lactobacillus brevis WiKim0069

Extension of the storage stability of freeze-dried lactic acid bacteria is important for industrialization. In this study, the effect of cation influx from soy powder, which contains high amounts of cations, as a cryoprotective agent on the viability of freeze-dried Lactobacillus brevis WiKim0069 was tested. Compared to that in the absence of the soy powder, bacterial viability was significantly higher in the presence of soy powder. Approximately 4.7% of L. brevis WiKim0069 survived in the
more » ... ce of the protective agent, whereas 92.8% viability was observed in the presence of soy powder. However, when cations were removed from the soy powder by using ethylenediaminetetraacetic acid (EDTA) and a cationic resin filter, the viability of L. brevis WiKim0069 decreased to 22.9–24.7%. When the soy powder was treated with ethylene glycol tetraacetic acid, the viability was higher than when it was pretreated with EDTA and a cationic resin filter, suggesting that Mg2+ had a role in enhancing the viability of L. brevis WiKim0069. Cold adaptation at 10 °C prior to freeze-drying had a positive effect on the storage stability of freeze-dried L. brevis WiKim0069, with 60.6% viability after 56 days of storage. A decrease in the fluorescence polarization value indicated an increase in membrane fluidity, which regulates the activity of ion channels present in the cell membrane. Cold adaptation caused activation of the cation channels, resulting in increased intracellular influx of cations, i.e., Ca2+ and Mg2+. These results suggest that cold adaptation can be used to improve the storage stability of L. brevis WiKim0069.
