Quantitative MRI assessment of leukoencephalopathy

Quantitative MRI assessment of leukoencephalopathy is difficult because the MRI properties of leukoencephalopathy significantly overlap those of normal tissue. This report describes the use of an automated procedure for longitudinal measurement of tissue volume and relaxation times to quantify leukoencephalopathy. Images derived by using this procedure in patients undergoing therapy for acute lymphoblastic leukemia (ALL) are presented. Five examinations from each of five volunteers (25
more &raquo; ... ons) were used to test the reproducibility of quantitated baseline and subsequent, normal-appearing images; the coefficients of variation were less than 2% for gray and white matter. Regions of leukoencephalopathy in patients were assessed by comparison with manual segmentation. Two radiologists manually segmented images from 15 randomly chosen MRI examinations that exhibited leukoencephalopathy. Kappa analyses showed that the two radiologists' interpretations were concordant (κ = 0.70) and that each radiologist's interpretations agreed with the results of the automated procedure (κ = 0.57 and 0.55).The clinical application of this method was illustrated by analysis of images from sequential MR examinations of two patients who developed leukoencephalopathy during treatment for ALL. The ultimate goal is to use these quantitative MR imaging measures to better understand therapy-induced neurotoxicity, which can be limited or even reversed with some combination of therapy adjustments and pharmacological and neurobehavioral interventions.

more &raquo; ... averaging method without changing the number of slices and then using a binary mask generated by a histogram threshold method to define the sampled voxels in the reduced image; (b) improvement of the efficiency of the correction function by using a Legendre polynomial as an orthogonal base function polynomial; (c) use of a nonparametric Parzen window estimator with a Gaussian kernel to calculate the probability density function and Shannon entropy directly from the image data; and (d) performing entropy minimization with a conjugate gradient method. Results showed that this algorithm could correct different types of MR images from different types of coils acquired at different field strengths very efficiently and with decreased computational load.

Introduction-Medical advances over the last several decades, including CNS prophylaxis, have greatly increased survival in children with leukemia. As survival rates have increased, clinicians and scientists have been afforded the opportunity to further develop treatments to improve the quality of life of survivors by minimizing the long-term adverse effects. When evaluating the effect of antileukemia therapy on the developing brain, magnetic resonance (MR) imaging has been the preferred

Method and results-Computer-aided detection of changes on neuroimages enables us to objectively differentiate leukoencephalopathy from normal maturation of the developing brain. Quantitative tissue segmentation algorithms and relaxometry measures have been used to determine the prevalence, extent, and intensity of white matter changes that occur during therapy. More recently, diffusion tensor imaging has been used to quantify microstructural changes in the integrity of the white matter fiber tracts. MR perfusion imaging can be used to noninvasively monitor vascular changes during therapy. Changes in quantitative MR measures have been associated, to some degree, with changes in neurocognitive function during and after treatment Conclusion-In this review, we present recent advances in quantitative evaluation of MR imaging and discuss how these methods hold the promise to further elucidate the pathophysiologic effects of treatment for childhood leukemia.

Polar effects of transposon insertion into a minimal bacterial genome

Global transposon mutagenesis is a valuable tool for identifying genes required for cell viability. Here we present a global analysis of the orientation of viable Tn5-PuroR inserts into the near-minimal bacterial genome of JCVI-syn2.0. Sixteen of the 478 protein coding genes show a noticeable asymmetry in the orientation of disrupting insertions of Tn5-PuroR. Ten of these are located in operons, upstream of essential or quasi-essential genes. Inserts transcribed in the same direction as the
more &raquo; ... stream gene are favored, permitting read-through transcription of the essential or quasi-essential gene. Some of these genes were classified as quasi-essential solely because of polar effects on the expression of downstream genes. Three genes showing asymmetry in Tn5-PuroR insertion orientation prefer the orientation that avoids collisions between read-through transcription of Tn5-PuroR and transcription of an adjacent gene. One gene (_0132) shows a strong preference for Tn5-PuroR insertions transcribed upstream, away from the downstream non-essential gene _0133. This suggested that expression of _0133 due to readthrough from the Tn5-PuroR is lethal when _0132 function is disrupted by transposon insertion. This led to the identification of genes _0133 and _0132 as a toxin-antitoxin pair. The three remaining genes show read-through transcription of Tn5-PuroR directed downstream, and away from sizable upstream intergenic regions (199-bp to 363-bp), for unknown reasons. In summary, polar effects of transposon insertion can, in a few cases, affect the classification of genes as essential, quasi-essential, or non-essential, and sometimes can give clues to gene function. IMPORTANCE In studies of the minimal genetic requirements for life we used global transposon mutagenesis to identify genes needed for a minimal bacterial genome. Transposon insertion can disrupt function of a gene but can also have polar effects on the expression of adjacent genes. In the Tn5-PuroR construct used in our studies read-through transcription from Tn5-PuroR can drive expression of downstream genes. This results in a preference for Tn5-PuroR insertions transcribed toward a downstream essential or quasi-essential gene within the same operon. Such polar effects can have an impact on the classification of genes as essential, quasi-essential, or non-essential, but this is observed in only a few cases. Also, polar effects of Tn5-PuroR insertion can sometimes give clues to gene function.
