Giant cell arteritis (GCA), also known as temporal arteritis or Horton disease, is categorized as a large- and medium-sized vessels vasculitis. Systemic symptoms are common in GCA and although vascular involvement may be widespread, the cranial branches of the aortic arch are responsible for the hallmark symptoms of GCA: headache, jaw claudication and ocular symptoms, particularly visual loss. The large vessel (LV)-GCA phenotype may differ or overlap from cranial arteritis. Clinical

Symptoms of polymyalgia rheumatica occurring in a patient with GCA include characteristic proximal polyarthralgias and myalgias, sometimes accompanied by remitting seronegative symmetrical synovitis with pitting edema (RS3PE), Less common manifestations reported include central nervous system involvement, audiovestibular and upper respiratory symptoms, pericarditis, mesenteric ischemia and female genital tract involvement.

AIM To evaluate the impact of the timing of capsule endoscopy (CE) in overt-obscure gastrointestinal bleeding (OGIB). METHODS Retrospective, single-center study, including patients submitted to CE in the setting of overt-OGIB between January 2005 and August 2017. Patients were divided into 3 groups according to the timing of CE (≤ 48 h; 48 h-14 d; ≥ 14 d). The diagnostic and therapeutic yield (DY and TY), the rebleeding rate and the time to rebleed were calculated and compared between groups.

Timing of capsule endoscopy in overt obscure GI compared. RESULTS One hundred and fifteen patients underwent CE for overt-OGIB. The DY was 80%, TY-46.1% and rebleeding rate -32.2%. At 1 year 17.8% of the patients had rebled. 33.9% of the patients performed CE in the first 48 h, 30.4% between 48h-14d and 35.7% after 14 d. The DY was similar between the 3 groups (P = 0.37). In the ≤ 48 h group, the TY was the highest (66.7% vs 40% vs 31.7%, P = 0.005) and the rebleeding rate was the lowest (15.4% vs 34.3% vs 46.3% P = 0.007). The time to rebleed was longer in the ≤ 48 h group when compared to the > 48 h groups (P = 0.03). CONCLUSION Performing CE within 48 h from overt-OGIB is associated to a higher TY and a lower rebleeding rate and longer time to rebleed.

RESUMO JUSTIFICATIVA E OBJETIVOS: A doença de Madelung, também conhecida como lipomatose simétrica benigna (LSB), é um tipo raro de lipodistrofia que se caracteriza por deposição simétrica de tecido adiposo no pescoço, porção superior do tronco e membros, determinando um desfiguramento progressivo do paciente. De etiologia desconhecida, a doença acomete principalmente homens, etilistas e, embora usualmente assintomática, pode causar complicações graves, tais como compressão traqueal, laríngea
more &raquo; ... mediastinal. O objetivo deste estudo foi relatar um caso de doença de Madelung, bem como sua possível patogênese e tratamento. RELATO DO CASO: Paciente do sexo masculino, 45 anos, etilista, cardiopata, com lipomas distribuídos simetricamente na região cervical e superior do tórax. Exames gerais sem alterações e tomografia cervical com diminuição discreta da luz traqueal. CONCLUSÃO: A LSB, relatada como doença benigna, pode originar massas volumosas e sintomas compressivos de vias aéreas , conferindo prognóstico reservado. Ressalta-se a importância da suspeição diagnóstica, que é feita à ectoscopia.

Leishmaniainfection consists in two sequential events, the host cell colonization followed by the proliferation/dissemination of the parasite. In this review, we discuss the importance of two distinct sets of molecules, the secreted and/or surface and the nonsecreted antigens. The importance of the immune response against secreted and surface antigens is noted in the establishment of the infection and we dissect the contribution of the nonsecreted antigens in the immunopathology associated with

As a further example of proteins belonging to these two different groups, we include several laboratorial observations onLeishmaniaSir2 andLicTXNPx as excreted/secreted proteins andLmS3arp andLimTXNPx as nonsecreted/panantigens. The role of these two groups of antigens in the immune response observed during the infection is discussed.

Operation Regimes: A Comparison Based on Nannochloropsis oceanica Biomass and Lipid Productivity

Microalgae are currently considered to be a promising feedstock for biodiesel production. However, significant research efforts are crucial to improve the current biomass and lipid productivities under real outdoor production conditions. In this context, batch, continuous and semi-continuous operation regimes were compared during the Spring/Summer seasons in 2.6 m3 tubular photobioreactors to select the most suitable one for the production of the oleaginous microalga Nannochloropsis oceanica.
more &raquo; ... sults obtained revealed that N. oceanica grown using the semi-continuous and continuous operation regimes enabled a 1.5-fold increase in biomass volumetric productivity compared to that cultivated in batch. The lipid productivity was 1.7-fold higher under semi-continuous cultivation than that under a batch operation regime. On the other hand, the semi-continuous and continuous operation regimes spent nearly the double amount of water compared to that of the batch regime. Interestingly, the biochemical profile of produced biomass using the different operation regimes was not affected regarding the contents of proteins, lipids and fatty acids. Overall, these results show that the semi-continuous operation regime is more suitable for the outdoor production of N. oceanica, significantly improving the biomass and lipid productivities at large-scale, which is a crucial factor for biodiesel production.

Oophoropexy to the Round Ligament after Recurrent Adnexal Torsion

Recurrent adnexal torsion is a rare gynecological emergency. We report a case of recurrent ipsilateral adnexal torsion in a woman with polycystic ovaries, previously submitted to a laparoscopic plication of the utero-ovarian ligament. Due to the recurrence after the plication of the utero-ovarian ligament, the authors performed a laparoscopic oophoropexy to the round ligament, which is an underreported procedure. The patient was asymptomatic for 1 year, after which she had a new recurrence and

Since then, she regained the quality of life without any gynecological symptoms.Oophoropexy to the round ligament may be considered when other techniques fail or, perhaps, as a first option in selected cases of adnexal torsion, as it may allow the prevention of recurrence without increasing morbidity while preserving the adnexa.

Nannochloropsis oceanica Cultivation in Pilot-Scale Raceway Ponds—From Design to Cultivation

Raceways ponds are the microalgal production systems most commonly used at industrial scale. In this work, two different raceway configurations were tested under the same processing conditions to compare their performance on the production of Nannochloropsis oceanica. Biomass productivity, biochemical composition of the produced biomass, and power requirements to operate those reactors were evaluated. Water depths of 0.20 and 0.13 m, and culture circulation velocities of 0.30 and 0.15 m s−1

A standard configuration, which had a full channel width paddlewheel, proved to be the most energy efficient, consuming less than half of the energy required by a modified configuration (had a half channel width paddlewheel). The later showed to have slightly higher productivity, not enough to offset the large difference in energetic consumption. Higher flow velocity (0.30 m s−1) led to a 1.7 g m−2 d−1 improvement of biomass productivity of the system, but it increased the energy consumption twice as compared to the 0.15 m s−1 flow velocity. The latter velocity showed to be the most productive in lipids. A water depth of 0.20 m was the most suitable option tested to cultivate microalgae, since it allowed a 54% energy saving. Therefore, a standard raceway pond using a flow velocity of 0.3 m s−1 with a 0.20 m water depth was the most efficient system for microalgal cultivation. Conversely, a flow velocity of 0.15 m s−1 was the most suitable to produce lipids.

Cytoreductive Surgery With Hyperthermic Intraperitoneal Chemotherapy: Analysis of Perioperative Risk Factors and Impact on Outcome

Cytoreductive surgery plus hyperthermic intraperitoneal chemotherapy (CRS + HIPEC) is an effective treatment option for appropriately selected patients with peritoneal carcinomatosis. Our aim was to analyze a multidisciplinary approach and to study the perioperative risk factors associated with morbidity and mortality. Methods We reviewed all patients who underwent CRS + HIPEC from January 2019 till December 2020 at our oncologic center. Patient demographics, risk scores, intraoperative
more &raquo; ... s, postoperative care, analgesia protocol, and adverse events (AE) within 30 days after treatment were collected and statistically analyzed. Results Of the 98 patients evaluated preoperatively by a multidisciplinary team, 39 patients required active optimization. The median age was 61 years, and 67 were women. Most tumors were appendiceal in origin. The median peritoneal cancer index (PCI) score was 12, and the median operative time length (OTL) was 400 minutes. Body mass index, Physiological and Operative Severity Score for the enUmeration of morbidity, PCI score, crystalloid volume, cell concentrates, and OTL were associated with postoperative intensive care unit admission (p <0.05). Epidural analgesia was given to 74 patients. AEs occurred in 39 patients, and 25 of the AEs were classified as mild or moderate. The intraoperative variables associated with development of AEs were anesthesia technique, estimated blood loss, crystalloid volume, cell concentrates, OTL, and analgesia protocol (p <0.05). On multivariate analysis, crystalloid volume >6 L, intravenous sufentanil analgesic protocol, and OTL were associated with 67%, 38%, and 15% increased risk of AE, respectively. Conclusion Our study highlighted the importance of a perioperative protocol with a standardized multidisciplinary approach in order to decrease the incidence of postoperative AE.
