Primary hepatic choriocarcinoma in a female patient

Choriocarcinoma is a germ cell tumor belonging to the spectrum of gestational trophoblastic diseases [1, 2] . It is characterized by wide and rapid hematogenous metastases, usually to the lungs and vagina [3] . It mostly occurs in the gonads and less frequently in extrag-onadal locations, including the mediastinum, retroperitoneum, stomach, or liver as a primary tumor. more » ... nowledge, nine case reports of primary hepatic choriocarcinoma have been published in international journals [4] [5] [6] [7] [8] . In this case report, we describe a patient with primary liver choriocarcinoma and lung metastases. CASE REPORT A 59-year-old woman was referred to our clinic with a history of upper right quadrant pain lasting 1 month. The patient's medical history was unremarkable. Computed tomography (CT) revealed a large hypervascular mass in the right liver (Fig. 1) . Magnetic resonance imaging revealed large masses with tiny daughter nodules in the right lobe of the liver, characteristic of angiosarcoma (Fig. 2) . Cervicovaginal smear was negative for intraepithelial lesions and malignancy. Blood tests on admission revealed a low hemoglobin level (6.5 g/dL). Choriocarcinoma is a germ cell tumor belonging to the spectrum of gestational trophoblastic diseases, and occurs most commonly in the gonads. Primary hepatic choriocarcinoma is very rare, especially in females. A 59-year-old woman was referred to our clinic with a history of upper right quadrant pain lasting 1 month. Computed tomography and magnetic resonance imaging revealed a large hypervascular mass in the right liver, characteristic of angiosarcoma. The patient underwent right hemihepatectomy without unexpected events. Histopathology revealed the tumor as a choriocarcinoma, with numerous syncytiotrophoblasts. Immunohistochemistry was positive for β-human chorionic gonadotropin and CK-19, with a Ki-67 labeling index of 90%. The tumor showed focal positivity for CD31 and human placental lactogen. The patient started chemotherapy (vincristine and cyclophosphamide) on postoperative day 16. The patient's clinical condition improved and she was discharged on postoperative day 17. Primary hepatic choriocarcinoma in female adults is a rare aggressive tumor, and further studies are needed to improve its diagnosis and treatment.

Evaluation of the Mechanical Properties of a 3D-Printed Mortar

The mechanical properties of 3D-printed mortars are determined in terms of their compressive and direct tensile bond strengths. To determine such properties using existing methods, a preliminary experiment was conducted. The compressive strength of the printed mortar was compared to mold-casted specimens and it was found that the compressive strength decreased by ~30%. When the specimens were fabricated using a specially designed stainless steel frame and core drill, followed by extraction and the application of physical impact, the 28 days compressive strength of the specimen decreased by ~50%.

more » ... is very rare and has only been reported once previously. A previously healthy 33-year-old woman presented with generalized edema, puffy hands, arthralgia, and Raynaud's phenomenon. We diagnosed MCD with proteinuria (> 9 g/day) and hypoalbuminemia (< 1.0 g/dL) by renal biopsy, and MCTD with a simultaneously high titer of the anti-U1 RNP antibody. The patient was treated with 200 mg/day cyclosporine and 30 mg/day prednisolone. The proteinuria and hypoalbuminemia normalized 2 months later. We report a patient with severe MCD associated with MCTD.

Neurotoxic and Neuroprotective Actions of Catecholamines in Cortical Neurons

We examined the possibility that catecholamines (CA) could act as endogenous modulators of neuronal death. Exposure to high doses (G100 M) of dopamine (DA) caused widespread neuronal death within 24 h in mouse cortical cell cultures and was accompanied by cell body shrinkage, aggregation and condensation of nuclear chromatin, and prominent internucleosomal DNA fragmentation. Epinephrine, but not norepinephrine (NE), was slightly toxic to neurons at doses higher than 1 mM. DA-induced death was
more » ... tenuated by the addition of three different anti-apoptosis agents, 1 g/ml cycloheximide, 25 mM K ؉ , or 100 ng/ml brainderived neurotrophic factor (BDNF). While treatment with 100 M N-acetyl-L-cysteine attenuated DA neurotoxicity, neither the glutamate antagonists (10 M MK-801 plus 50 M CNQX) nor several antioxidants [trolox, 100 M; Mn (III) tetrakis (4-benzoic acid) porphyrin chloride, 100 M; Mn (III) tetrakis (1-methyl-4pyridyl) prophyrin pentachloride, 100 M; N-tert-butyl-␣-phenylnitrone, 3 mM] prevented the CA-induced apoptosis. Interestingly, all CA at 1-30 M attenuated free radical-mediated neuronal necrosis following exposure to 30 M Fe 2؉ or 200 M H 2 O 2 , which was insensitive to DA or NE antagonists. Like trolox, CA reduced levels of the stable free radical 1,1-diphenyl-2picrylhydrazyl under cell-free conditions, raising the possibility that CA as an antioxidant protects neurons. We also found that the neuroprotective effect of CA prolonged the protective effects of BDNF against serum deprivation. The present findings suggest that CA induces apoptosis at high doses but prevents free radical-mediated neurotoxicity as an anti-oxidant without being coupled to the receptors. 1999 Academic Press

Primary pulmonary T-cell lymphoma is an extremely rare malady, and we diagnosed this in a 52-year-old male who was admitted to our hospital with cough for the previous two weeks. The follow-up CT scan showed an increased extent of the multifocal consolidative lesions despite that the patient had undergone chemotherapy. rimary lymphoma of the lung is a rare disorder, and primary pulmonary lymphoma represents only 0.5-1% of all primary pulmonary malignancies, less than 1% of all the cases of non-Hodgkin's lymphoma (NHL) and 3-4% of all the extranodal manifestations of NHL (1). Most of the cases of the primary lymphoma of the lung originate from the B-cell lineage, and the disease is frequently located in the bronchus-associated lymphoid tissue. Very few cases of pulmonary T-cell lymphoma have been reported and the imaging features of this rare cancer have not been well characterized. We report here on an extremely rare case of primary pulmonary peripheral T-cell lymphoma not otherwise specified (PTCLN), and we present the CT and positron emission tomography (PET) images. CASE REPORT A 52-year-old male was admitted to our hospital suffering with cough, fever and sweating for the previous two weeks. There was no history of treatment or medication for diabetes mellitus and hypertension. The patient was a smoker (45 pack-years) and he drank alcohol. On the posteroanterior chest radiograph, variable sized nodules and masses were seen in both lower lobes and the right upper lobe (Fig. 1A) . The contrast enhanced chest CT scan revealed variable sized masses in the right upper lobe and both lower lobes, and the masses displayed central necrosis (Fig. 1B-D) . A small fluid collection was seen in the left hemithorax. No endobronchial lesion was visualized on bronchoscopy. Impact of diabetes mellitus on mortality in patients with acute heart failure: a prospective cohort study

Although more than one-third of the patients with acute heart failure (AHF) have diabetes mellitus (DM), it is unclear if DM has an adverse impact on clinical outcomes. This study compared the outcomes in patients hospitalized for AHF stratified by DM and left ventricular ejection fraction (LVEF). more » ... aluated the impact of DM on these endpoints according to HF subtypes and glycemic control. During a median follow-up of 3.5 years, there were 235 (4.4%) in-hospital mortalities and 2500 (46.3%) overall mortalities. DM was significantly associated with increased overall mortality after adjusting for potential confounders (adjusted hazard ratio [HR] 1.11, 95% confidence interval [CI] 1.03-1.22). In the subgroup analysis, DM was associated with higher a risk of overall mortality in heart failure with reduced ejection fraction (HFrEF) only (adjusted HR 1.14, 95% CI 1.02-1.27). Inadequate glycemic control (HbA1c ≥ 7.0% within 1 year after discharge) was significantly associated with a higher risk of overall mortality compared with adequate glycemic control (HbA1c < 7.0%) (44.0% vs. 36.8%, log-rank p = 0.016). DM is associated with a higher risk of overall mortality in AHF, especially HFrEF. Well-controlled diabetes (HbA1c < 7.0%) is associated with a lower risk of overall mortality compared to uncontrolled diabetes. Trial registration ClinicalTrial.gov, NCT01389843. Schwannoma is a benign neurogenic tumor arising from the nerve sheath, and it presents as a well defined mass. Isolated gastrointestinal schwannoma is a relatively rare finding and schwannoma of the appendix is extremely rare. We report here on a case of schwannoma that arose from the appendix, and this lesion was pathologically confirmed.

Parameteric Assessment of Water Use Vulnerability of South Korea using SWAT model and TOPSIS

This study assessed the water use vulnerability for 12 basins of South Korea. The annual runoff of 12 basins are derived using a Soil and Water Assessment Tool (SWAT) and the calculated runoff per unit area and population are compared with each basin. The 18 indicators are selected in order to assess the vulnerability. Those are classified by aspects of demand, loss and supply of water use. The results show that the water availability vulnerability of Hyeongsan River has the highest value followed by Sapgyo River; Dongjin River; Seomjin River; Anseong River; Mangyung River; Nakdong River; Tamjin River; Youngsan River, Geum River; Taehwa River; and Han River. The result of this study has a capability to provide references for the index deveopment of climate change vulnerability assessment.

Dependency on Smartphone Use and its Association with Anxiety in Korea

Objective. South Korea has the highest rate of smartphone ownership worldwide, which is a potential concern given that smartphone dependency may have deleterious effects on health. We investigated the relationship between smartphone dependency and anxiety. Methods. Participants included 1,236 smartphone-using students (725 men and 511 women) from six universities in Suwon, South Korea. more » ... (i.e., demographic, health-related, and socioeconomic characteristics). To measure smartphone dependency and anxiety, we used questionnaires of Yang's test developed from Young's Internet Addiction Test and Zung's Self-Rating Anxiety Scale. We used multiple logistic regression to determine the association between smartphone dependency and anxiety after adjusting for relevant factors. Results. On a scale from 25 to 100, with higher scores on the smartphone dependency test indicating greater dependency, women were significantly more dependent on smartphones than were men (mean smartphone dependency score: 50.7 vs. 56.0 for men and women, respectively, p,0.001). However, the amount of time spent using smartphones and the purpose of smartphone use affected smartphone dependency in both men and women. Particularly, when daily use time increased, smartphone dependency showed an increasing trend. Compared with times of use ,2 hours vs. $6 hours, men scored 46.2 and 56.0 on the smartphone dependency test, while women scored 48.0 and 60.4, respectively (p,0.001). Finally, for both men and women, increases in smartphone dependency were associated with increased anxiety scores. With each one-point increase in smartphone dependency score, the risk of abnormal anxiety in men and women increased by 10.1% and 9.2%, respectively (p,0.001). Conclusion. Among this group of university students in South Korea, smartphone dependency appeared to be associated with increased anxiety. Standards for smartphone use might help prevent deleterious health effects.

Pulmonary bronchogenic cyst in adults is rare and the typical appearance is a sharply circumscribed, round or oval nodule or mass, usually in the medial third of the lungs. Bronchial polyps are rare histopathologically distinct nonneoplastic endobronchial lesions and are classified as multiple papillomas, solitary papillomas, and inflammatory polyps. more » ... f four weeks' duration. Chest radiography revealed a cystic lesion in the right upper lung. Computed tomography revealed a 4× ×5 cm sized large cyst. Neither enlarged mediastinal lymph nodes nor extrabronchial involvements were observed. Flexible bronchoscopy revealed a peduncular polyp about 2 cm in length originating from the anterior segment of right upper lung. After bronchoscopic removal of polyp, cystic lesion of the right upper lung disappeared.
