Kontakt: PsyDok Summary Outpatient Care for Child and Adolescent Psychiatric Disorders -Data from an Insuree-Related Epidemiological Study he aim of the study was to examine the extent to which psychiatric disorders are documented as a diagnosis requiring treatment in children and adolescents as well as to investigate the frequency with which non-medicinal interventions are utilized. he database comprised an 18.75 % random sample of insurees from the AOK statutory health insurance fund Hessen. he data allowed an examination of the care-seeking behaviour of 55,545 children and adolescents aged 0 to 18 years in the year 2006. With respect to all psychiatric diagnostic categories, a psychiatric diagnosis was documented for 19.3 % of children and adolescents in 2006. 27 % of children and adolescents had received more than one psychiatric diagnosis. Up to the age of 14 years, psychiatric diagnoses were more frequently documented for boys than for girls, while this pattern was reversed from the age of 15 years upwards. Typical diferences in the spectrum of services received were evident depending on the psychiatric diagnosis made. Younger age groups predominantly received logopedic treatment, occupational therapy, and physiotherapy/therapeutic exercise. With increasing age, the utilization of psychiatric and psychotherapeutic services expanded. Outpatient care is primarily provided by pediatricians and general physicians followed by practice-based child and adolescent psychiatrists as well as outpatient child and adolescent psychiatric clinics. While not all diagnoses necessitate psychiatric or psychotherapeutic interventions, it may be assumed that the predominant seeking of care from pediatricians and general physicians is relective of continued inadequate access to child and adolescent psychiatrists or other specialized facilities and professional groups. Alpha-Xenorhabdolysins (Xax) are alpha-pore-forming toxins (alpha-PFT) from pathogenic bacteria that form 1-1.3 MDa large pore complexes to perforate the host cell membrane. PFTs are used by a variety of bacterial pathogens as an offensive or defensive mechanism to attack host cells. ... rage resolution of 0.4 nm and the crystal structures of the soluble monomers of XaxA and XaxB at 0.25 nm and 0.34 nm, respectively. The structures reveal that XaxA and XaxB are built similarly and appear as heterodimers in the 12-15 subunits con-taining pore. The structure of the XaxAB pore represents therefore the first structure of a bi-component alpha-PFT. Major conformational changes in XaxB, including the swinging out of an amphipathic helix are responsible for membrane insertion. XaxA acts as an activator and stabilizer for XaxB that forms the actual transmembrane pore. Based on our results, we propose a novel structural model for the mechanism of action of Xax toxins.

Abstract Theoretical considerations suggest that nonlinear health care price schedules have heterogeneous effects on health care demand. In this paper, we develop and apply a finite mixture bivariate probit model to analyze whether there are heterogeneous reactions to the introduction of a nonlinear price schedule in the German statutory health insurance system. In administrative insurance claims data from the largest German health insurance plan, we find that some individuals strongly react to the new price schedule while a second group of individuals does not react. Post-estimation analyses reveal that the group of the individuals who do not react to the reform includes the relatively sick. These results are in line with forward-looking behavior: Individuals who are already sick expect that they will hit the kink in the price schedule and thus are less sensitive to the co-payment.
