Choroidal neovascularization (CNV) is aberrant angiogenesis associated with exudative age-related macular degeneration (AMD), a leading cause of blindness in the elderly. ... Ikk2 conditional knockout mice or an IKK2 inhibitor. The retinal neuronal tissue and RPE deletion of IKK2 was generated by breeding Ikk2 2/flox mice with Nestin-Cre mice. Deletion of Ikk2 in the retina caused no obvious defect in retinal development or function, but resulted in a significant reduction in laser-induced CNV. In addition, intravitreal or retrobulbar injection of an IKK2 specific chemical inhibitor, TPCA-1, also showed similar inhibition of CNV. Furthermore, in vitro inhibition of IKK2 in ARPE-19 cells significantly reduced heat shock-induced expression of NFKBIA, IL1B, CCL2, VEGFA, PDGFA, HIF1A, and MMP-2, suggesting that IKK2 may regulate multiple molecular pathways involved in laser-induced CNV. The in vivo laser-induced expression of VEGFA, and HIF1A in RPE and choroidal tissue was also blocked by TPCA-1 treatment. Thus, IKK2/NF-kB signaling appears responsible for production of pro-inflammatory and pro-angiogenic factors in laser-induced CNV, suggesting that this intracellular pathway may serve as an important therapeutic target for aberrant angiogenesis in exudative AMD.

There has been work to solve this challenge based on extracting shape and contrast features and then applying classification models on those features. The limiting factor in this existing work is the use of hand crafted filters on the image such as Gabor or Sobel filters or Haar features. These hand crafted methods rely on domain knowledge to construct. We would like to learn the optimal filters and features based on training examples. In order to dynamically learn filters and features we look to Convolutional Neural Networks (CNNs) which have shown their dominance in computer vision. The power of CNNs is that they can learn image filters which generate features for high accuracy classification.

We propose a novel variant of SGD customized for training network architectures that support anytime behavior: such networks produce a series of increasingly accurate outputs over time. Efficient architectural designs for these networks focus on re-using internal state; subnetworks must produce representations relevant for both immediate prediction as well as refinement by subsequent network stages. We consider traditional branched networks as well as a new class of recursively nested networks.

... Our new optimizer, Orthogonalized SGD, dynamically re-balances task-specific gradients when training a multitask network. In the context of anytime architectures, this optimizer projects gradients from later outputs onto a parameter subspace that does not interfere with those from earlier outputs. Experiments demonstrate that training with Orthogonalized SGD significantly improves generalization accuracy of anytime networks.

We show that trapezoids with identical Neumann spectra are congruent up to rigid motions of the plane. The proof is based on heat trace invariants and some new wave trace invariants associated to certain diffractive billiard trajectories. The reason we can only treat the Neumann case is that the wave trace is "more singular" for the Neumann case compared to the Dirichlet case. This is a new observation which is interesting on its own.

L'examen clinique montre une sarcopénie importante, un status opératoire sans complication (notamment sans signe de surinfection) et l'absence de signe de décompensation cardiaque. Le laboratoire révèle une légère anémie normochrome normocytaire à 110 g/l, un syndrome inflammatoire avec des leucocytes à 12 G/l et une protéine C réactive à 150 mg/l, des électrolytes dans la norme, une insuffisance rénale aiguë fonctionnelle avec une créatinine à 150 µmol/l et une hormone de stimulation de la thyroïde (TSH) abaissée à 0,1 mU/l (normes: 0,27-4,2 mU/l). Suite au contact avec son médecin traitant, vous apprenez que la fibrillation auriculaire était déjà connue, en revanche le dernier dosage de la TSH était dans la norme. Question 1: Concernant la TSH, laquelle des propositions suivantes est erronée? a) La TSH a une sensibilité et une spécificité supérieures à la L-tétraiodothyronine libre (T4 libre, ou T4L) dans le dépistage des dysthyroïdies b) Les valeurs de référence de la TSH sont établies par chaque laboratoire à partir des valeurs d'une population locale sans pathologies thyroïdiennes connues c) Le prix d'un dosage de la TSH est d'approximativement CHF 10 d) La TSH est sécrétée selon un rythme circadien, avec des taux plus élevés le matin e) Certains médicaments, tels que les corticostéroïdes, influencent la sécrétion de la TSH La L-tétraiodothyronine (T4L) est moins sensible et spécifique que la TSH pour le dépistage des dysthyroïdies [1]. La TSH est sécrétée de façon circadienne, avec un pic de sécrétion vers minuit. Le dosage de la TSH peut être effectué à tout moment de la journée, les taux étant stables sur la journée. Une anamnèse médicamenteuse précise est primordiale en cas de dysthy-roïdie, car de nombreux médicaments influencent la sécrétion des hormones thyroïdiennes. Ainsi, les corticostéroïdes, les anti-inflammatoires, l'héparine ou encore le métoclopramide, traitements fréquemment administrés dans un contexte per-opératoire, influencent le bilan thyroïdien [2]. Revenons à notre patiente: devant une TSH abaissée, une hyperthyroïdie primaire est suspectée. Question 2: Quel bilan souhaitez-vous effectuer? a) Dosage de la T4L b) Dosage de la T4L et de la L-triiodothyronine (T3 libre, ou T3L) c) Dosage de la T4L et de la T4 totale (T4T) d) Dosage de la T3 inverse (reverse T3, ou rT3) e) Dosage d'anticorps anti-récepteur de TSH (TRAK), anti-thyroperoxydase (anti-TPO) et anti-thyroglobuline (anti-Tg) Les hormones thyroïdiennes sont en majorité liées à des protéines plasmatiques: l'albumine, la globuline liant la thyroxine (TBG) et la transthyrétine (TTR). Seules les fractions libres (<1%) sont biologiquement actives. La concentration des hormones thyroïdiennes totales variant en fonction de celles des protéines plasmatiques, il est recommandé de doser les fractions libres. En cas de suspicion d'une hyperthyroïdie, un dosage de T4L et T3L doit être effectué [3]. Le dosage de la T4L seule n'est pas suffisant, car parfois seule la T3L est augmentée. Le dosage des auto-anticorps est à discuter dans un deuxième temps en cas de confirmation d'une hyperthyroïdie. Le dosage de la 3-3'-5'-triiodothyronine, reverse t3 (rT3) ne doit pas être réalisé de routine. Chez notre patiente, un dosage de la T4L et de la T3L est effectué: T4L à 6,5 pmol/l (normes: 12-22 pmol/l) et T3L à 2 pmol/l (normes: 3,1-6,8 pmol/l). Question 3: Quel diagnostic vous-semble le plus probable? a) Une hypothyroïdie infraclinique b) Une hypothyroïdie primaire c) Une hypothyroïdie secondaire d) Un syndrome euthyroïdien e) Une thyroïdite de De Quervain Au vu du contexte opératoire récent, de la dénutrition et de l'association entre une TSH, une T4L et une T3L abaissées, le diagnostic le plus probable chez cette patiente est un syndrome euthyroïdien (SE). In einem Gespräch mit ihrem behandelnden Arzt erfahren Sie, dass das Vorhofflimmern bereits bekannt ist, die letzte TSH-Bestimmung jedoch im Normbereich lag. Frage 1: Welche der folgenden Aussagen zum TSH-Wert ist falsch? a) Der TSH-Wert weist beim Screening auf Schilddrüsenerkrankungen eine höhere Sensitivität und Spezifizität auf als das freie Tetrajodthyronin (freies T4 oder fT4) b) Die TSH-Referenzwerte werden von jedem Labor anhand der Werte einer lokalen Population ohne bekannte Schilddrüsenerkrankungen festgelegt c) Die Kosten für eine TSH-Wert-Bestimmung betragen etwa 10 CHF d) TSH wird in einem zirkadianen Rhythmus mit höheren morgendlichen Werten ausgeschüttet e) Bestimmte Medikamente, wie Kortikosteroide, beeinflussen die TSH-Ausschüttung Tetrajodthyronin (fT4) weist beim Screening auf Schilddrüsenerkrankungen eine geringere Sensitivität und Spezifizität auf als der TSH-Wert [1]. TSH wird in einem zirkadianen Rhythmus mit Spitzenwerten gegen Mitternacht ausgeschüttet. Der TSH-Wert kann zu jeder Tageszeit bestimmt werden, da er tagsüber stabil ist. Bei einer Schilddrüsenerkrankung ist eine genaue Medikamentenanalyse unerlässlich, da zahlreiche Medikamente die Ausschüttung der Schilddrüsenhormone beeinflussen. Dies ist zum Beispiel der Fall bei Kortikosteroiden, Entzündungshemmern, Heparin oder Meto clopramid, die häufig bei Operationen eingesetzt werden, [2]. Bei unserer Patientin besteht aufgrund des niedrigen TSH-Werts der Verdacht auf eine primäre Hyperthyreose. Frage 2: Welche Laboruntersuchungen sollten Sie veranlassen? a) Bestimmung des fT4-Werts b) Bestimmung des fT4-Werts und des freien Trijodthyronins (freies T3 oder fT3) c) Bestimmung des fT4-Werts und des Gesamt-T4 (gT4) d) Bestimmung des Reverse T3 (rT3) e) Bestimmung der TSH-Rezeptor-(TRAK), Thyreoperoxidase-(TPO-AK) und Thyreoglobulin-Antikörper (Tg-AK) Schilddrüsenhormone sind grösstenteils an Plasmaproteine wie Albumin, Thyroxin-bindendes Globulin (TBG) und Transthyretin (TTR) gebunden. Lediglich die freien Hormone (<1%) sind biologisch aktiv. Da die Gesamt-Schilddrüsenhormonkonzentration von der Konzentration der Plasmaproteine abhängt, wird empfohlen, die freien Hormone zu bestimmen. Bei einem Hyperthyreoseverdacht sollten der fT4-und der fT3-Wert bestimmt werden [3]. Die ausschliessliche fT4-Bestimmung ist unzureichend, da mitunter nur das fT3 erhöht ist. Bei einer Bestätigung der Hyperthyreose kann zu einem späteren Zeitpunkt die Bestimmung von Autoantikörpern erwogen werden. Eine Bestimmung des 3-3'-5'-Triiod-L-thyronins und des Reverse T3 (rT3) gehört nicht zu den Routineuntersuchungen. Bei unserer Patientin werden der fT4-und der fT3-Wert bestimmt: fT4: 6,5 pmol/l (Normwerte: 12-22 pmol/l) und fT3: 2 pmol/l (Normwerte: 3,1-6,8 pmol/l).
