Multiple Sclerosis

Pathology The pathological conception of MS as a disseminated "plaque-like sclerosis" was established ~150 years ago; indeed, the demonstration of dissemination -in space (diseaserelated changes in multiple CNS regions, including white matter, gray matter, brainstem, spinal cord, and optic nerve; Figure 1 ) and time -forms the cornerstone of MS diagnosis. Our understanding of the details of that pathology, and especially how it evolves over time,

Anesthetic Management of Nasopharyngeal Angiofibroma Resection with Carotid Invasion in a Pediatric Patient
2016
OALib
The nasopharyngeal angiofibroma is an aggressive, vascular tumor that may represent a challenge for the anesthesiologist by the risk of profuse bleeding. We report the case of a 13 years old, 52 kg, ASA1, who was scheduled for the resection of an angiofibroma invading facial sinuses and internal carotid artery, and submitted to external carotid and internal maxillary arteries embolization one day before surgery. Mallampati 2, previous successful intubation reported. Induction was made with

... fol, fentanyl, lidocaine and rocuronium, followed by intubation by direct laringoscopy. Sevoflurane and remifentanyl were used for maintenance. Central venous access and arterial catheterization were provided. Inicial arterial gasometry was normal. During tumor resection close to the ethmoidal cells, there was profuse bleeding that required intensive volemic resuscitation and hemotransfusion. The cavernous part of the angiofibroma was resected by a neurosurgery team. During the whole procedure, the patient received 3500 ml of Saline Solution 0.9%, 4000 ml of RL, 1000 ml of hydroxyethyl starch, four RBC units, two plasma units and 4 platelets units. Serial arterial blood gas analysis revealed Hb = 6 g/dL as the minor value of hemoglobin and pH was physiological all the time. Fenylephrine, adrenaline and noradrenaline were infused. Even after the massive volume restitution, responsiveness to hydration was verified by the delta pulse pressure curve. Total diuresis was 300 ml. The total duration of the procedure was 9 hours. The patient was sent to the ICU after a tracheostomy. Transthoracic US revealed collapsed cava vein leading to new volume resuscitation.
Spiroplasma species as a rare cause of congenital cataract and uveitis: a case series
2021
BMC Ophthalmology
Background To date, only four cases of ocular spiroplasma infection have been reported in the entire ophthalmic literature. We add two more cases to raise awareness of this sight-threatening congenital disease that manifests as cataract with ocular inflammation. Case presentation Both infants were referred for cataracts associated with ocular inflammation. Case 1, a 3-week-old neonate presented with unilateral cataract, ocular inflammation and elevated intraocular pressure. Case 2 was a

... old infant with bilateral cataract and panuveitis. Lensectomies with or without vitrectomy and subsequent analyses of the specimens were performed. Transmission electron microscopy and multiplex polymerase chain reaction or 16 s rRNA gene polymerase chain reaction revealed spiroplasma species. Conclusions Spiroplasma as a very rare cause for congenital cataract might be underdiagnosed. We recommend performing polymerase chain reaction to probe for spiroplasma species in congenital cataracts with an inflammatory component.

Genetic and biochemical analysis of the fission yeast ribonucleoprotein particle containing a homolog of Srp54p
1994
Nucleic Acids Research
Mammalian signal recognition particle (SRP), a complex of six polypeptides and one 7SL RNA molecule, is required for targeting nascent presecretory proteins to the endoplasmic reticulum (ER). Earlier work identified a Schizosaccharomyces pombe homolog of human SRP RNA and showed that it is a component of a particle similar in size and biochemical properties to mammalian SRP. The recent cloning of the gene encoding a fission yeast protein homologous to Srp54p has made possible further

... ation of the subunit structure, subcellular distribution, and assembly of fission yeast SRP. S.pombe SRP RNA and Srp54p co-sediment on a sucrose velocity gradient and coimmunoprecipitate, indicating that they reside in the same complex. In vitro assays demonstrate that fission yeast Srp54p binds under stringent conditions to E.coli SRP RNA, which consists essentially of domain IV, but not to the full-length cognate RNA nor to an RNA in which domain III has been deleted in an effort to mirror the structure of bacterial homologs. Moreover, the association of S.pombe Srp54p with SRP RNA in vivo is disrupted by conditional mutations not only in domain IV, which contains its binding site, but in domains I and III, suggesting that the particle may assemble cooperatively. The growth defects conferred by mutations throughout SRP RNA can be suppressed by overexpression of Srp54p, and the degree to which growth is restored correlates inversely with the severity of the reduction in protein binding. Conditional mutations in SRP RNA also reduce its sedimentation with the ribosome/membrane pellet during cell fractionation. Finally, immunoprecipitation under native conditions of an SRP-enriched fraction from [ 35 S]-labeled fission yeast cells suggests that five additional polypeptides are complexed with Srp54p; each of these proteins is similar in size to a constituent of mammalian SRP, implying that the subunit structure of this ribonucleoprotein is conserved over vast evolutionary distances.

Prominent Oncogenic Roles of EVI1 in Breast Carcinoma
2017
Cancer Research
Overexpression of the EVI1 oncogene is associated typically with aggressive myeloid leukemia, but is also detectable in breast carcinoma where its contributions are unexplored. Analyzing a tissue microarray of 608 breast carcinoma patient specimens, we documented EVI1 overexpression in both estrogen receptor-positive (ER þ ) and estrogen receptor-negative (ER À ) breast carcinomas. Here, we report prognostic relevance of EVI1 overexpression in triple-negative breast carcinoma but not in the

... -positive breast carcinoma subset. In human breast cancer cells, EVI1 silencing reduced proliferation, apoptosis resistance, and tumorigenicity, effects rescued by estrogen supplementation in ER þ breast carcinoma cells. Estrogen addition restored ERK phosphorylation in EVI1-silenced cells, suggesting that EVI1 and estradiol signaling merge in MAPK activation. Conversely, EVI1 silencing had no effect on constitutive ERK activity in HER2 þ breast carcinoma cells. Microarray analyses revealed G-protein-coupled receptor (GPR) signaling as a prominent EVI1 effector mechanism in breast carcinoma. Among others, the GPR54-ligand KISS1 was identified as a direct transcriptional target of EVI1, which together with other EVI1-dependent cell motility factors such as RHOJ regulated breast carcinoma cell migration. Overall, our results establish the oncogenic contributions of EVI1 in ER-and HER2-negative subsets of breast cancer. Cancer Res; 77(8); 2148-60. Ó2017 AACR.
