This applied research paper introduces a novel framework for integrating hardware security and blockchain functionality with grid-edge devices to establish a distributed cyber-security mechanism that verifies the provenance of messages to and from the devices. Expanding the idea of Two Factor Authentication and Hardware Root of Trust, this work describes the development of a Cryptographic Trust Center(TM) (CTC(TM)) chip integrated into grid-edge devices to create uniform cryptographic key

Product managers, energy system designers, and security architects can utilize this modular framework as a unified approach to manage distributed devices of various vendors, vintages, and sizes. Results demonstrate the application of CTC(TM) to a blockchain-based Transactive Energy (TE) platform for provisioning of cryptographic keys and improved uniformity of the operational network and data management. This process of configuring, installing, and maintaining keys is described as Eco-Secure Provisioning(TM) (ESP(TM)). Laboratory test results show the approach can resolve several cyber-security gaps in common blockchain frameworks such as Hyperledger Fabric.

Screening for venous thromboembolism in patients with COVID-19

AbstractPulmonary thromboembolism and deep venous thrombosis occur frequently in hospitalised patients with COVID-19, the prevalence increases on the intensive care unit (ICU) and is very high in patients on extracorporeal membrane oxygenation (ECMO). We undertook a literature review to assess the usefulness of screening for peripheral venous thrombosis or pulmonary thrombosis in patients admitted with COVID-19. more » ... m baseline should alert the need for doppler ultrasound scan of the lower limbs. In the ICU setting, consideration should be given to routine screening with doppler ultrasound, given the high prevalence of thrombosis in this cohort despite standard anticoagulant thromboprophylaxis. However, absence of lower limb thrombosis on ultrasound does not exclude pulmonary venous thrombosis. Screening with CT pulmonary angiography (CTPA) is not justified in patients on the general wards, unless there are clinical features and/or marked elevations in markers of COVID-19-associated coagulopathy. However, the risk of pulmonary embolism or pulmonary thrombosis in ICU patients is very high, especially in patients on ECMO, where studies that employed routine screening for thrombosis with CT scanning have uncovered up to 100% incidence of pulmonary thrombosis despite standard anticoagulant thromboprophylaxis. Therefore, in patients at low bleeding risk and high clinical suspicion of venous thromboembolism, therapeutic anticoagulation should be considered even before screening, Our review highlights the need for increased vigilance for VTE, with a low threshold for doppler ultrasound and CTPA in high risk in-patient cohorts, where clinical features and D-dimer levels may not accurately reflect the occurrence of pulmonary thromboembolism.

Abstract Background: Single photodynamic therapy (PDT) has been effective in initial periodontal therapy, but only improved bleeding on probing (BoP) in maintenance patients after a single use. Repeated PDT has not been addressed. Objectives: To study the possible added benefits of repeated adjunctive PDT to conventional treatment of
more » ... l pockets in patients enrolled in periodontal maintenance. Material and Methods: Ten maintenance patients with 70 residual pockets [probing pocket depth (PPD)X5 mm] were randomly assigned for treatment five times in 2 weeks (Days 0, 1, 2, 7, 14) with PDT (test) or non-activated laser (control) following debridement. The primary outcome variable was PPD, and the secondary variables were clinical attachment level (CAL) and BoP. These were assessed at 3, 6 and 12 months following the interventions. Results: Greater PPD reductions were observed in the test (À 0.67 AE 0.34; p 5 0.01) compared with the control patients (À 0.04 AE 0.33; NS) after 6 months. Significant CAL gain (10.52 AE 0.31; p 5 0.01) was noted for the test, but not in the control (À 0.27 AE 0.52; NS) patients after 6 months. BoP percentages dcreased significantly in test (97-64%, 67%, 77%), but not control patients after 3, 6 and 12 months. Conclusions: Repeated (five times) PDT adjunctive to debridement yielded improved clinical outcomes in residual pockets in maintenance patients. The effects were best documented after 6 months.
