Structural mechanisms in NLR inflammasome signaling

Members of the NOD-like receptor (NLR) family mediate the innate immune response to a wide range of pathogens, tissue damage and other cellular stresses. They achieve modulation of these signals by forming oligomeric signaling platforms, which in analogy to the apoptosome are predicted to adopt a defined oligomeric architecture and will here be referred to as NLR oligomers. Once formed, oligomers of the NLR proteins NLRP3 or NLRC4 'recruit' the adaptor protein ASC and the effector caspase-1,
more » ... ector caspase-1, whereby NLRC4 can also directly interact with caspase-1. This results in large multi-protein assemblies, termed inflammasomes. Ultimately, the formation of these inflammasomes leads to the activation of caspase-1, which then processes the cytokines IL-1β and IL-18 triggering the immune response. Here we review new insights into NLR structure and implications on NLR oligomer formation as well as the nature of multi-protein inflammasomes. Of note, so dubbed 'canonical inflammasomes' can also be triggered by the NLR NLRP1b and the non-NLR protein AIM2, however the most detailed mechanistic information at hand pertains to NLRC4 while NLRP3 represents the quintessential inflammasome trigger. Thus these two NLRs are mainly used as examples in this article.

Leukocytes influence peripheral tissue oxygenation and perfusion in neonates

Background. Leukocyte counts may influence peripheral (micro) circulation due to changes in rheology. The aim of this study was to investigate a possible association between leukocyte counts and peripheral tissue oxygenation/perfusion measured with near infrared spectroscopy (NIRS) in term and preterm neonates. more » ... d and blood samples (leukocytes and C reactive protein (CRP)) taken to investigate clinical signs of infection. Tissue-oxygenation index (TOI), fractional oxygen extraction (FTEO), oxygen delivery (DO 2 ), oxygen consumption (VO 2 ) and vascular resistance (VR) were measured by NIRS and venous occlusion method. TOI, FTOE, DO 2 , VO 2 and VR were correlated to leukocyte counts on the same day and maximal CRP levels within 24 hours (CRP max). Results. In 180 infants, with a mean gestational age of 35.5±3.3 weeks, leukocyte counts were 16546± 8830/ l (median 14830; range 1790 to 67840) and CRP max was 8.0± 19.0 mg/l (median 0.0; range 0.0 to 110.0mg/l). TOI was 71.1±5.5%, FTOE 28.5±6.1%, DO 2 46.7±19.7, p= 0.005) with TOI and positively (r=0.17; p=0.029) with VR. Correlations with CRP max did not reach significance. Conclusion. We demonstrated that peripheral tissue oxygen consumption decreases and vascular resistance increases with increasing leukocyte counts.

Cushing's Syndrome in a Morbidly Obese Patient Undergoing Evaluation before Bariatric Surgery

Objective: Cushing's syndrome (CS) is extremely rare in morbidly obese patients. To date, no occurrences in obese patients with BMI above 60 kg/m 2 have been reported in the literature. Case Report: This case report describes a patient who was admitted to the ward of the Clinical Division of Endocrinology and Metabolism of the Medical University of Vienna in preparation for bariatric surgery. The patient was a 49-year-old female who showed morbid obesity (BMI 61.6 kg/m 2 ), hypertension, and
more » ... ypertension, and substituted hypothyroidism. Preoperative work-up revealed CS due to an adrenal adenoma. Therefore, the patient underwent unilateral adrenalectomy followed by bariatric surgery 6 months later. Conclusion: Since undiagnosed CS might result in severe perioperative complications in a population already at increased risk, this case report underlines the importance of careful endocrine evaluation of morbidly obese patients. After all, even rare endocrine causes should be excluded.

Limitations of Ensemble Bayesian Model Averaging for forecasting social science problems

We compare the accuracy of simple unweighted averages and Ensemble Bayesian Model Averaging (EBMA) to combining forecasts in the social sciences. A review of prior studies from the domain of economic forecasting finds that the simple average was more accurate than EBMA in four out of five studies. On average, the error of EBMA was 5% higher than the error of the simple average. A reanalysis and extension of a published study provides further evidence for US presidential election forecasting.

In addition, simple averages provide accurate forecasts in many settings. Researchers who develop new approaches to combining forecasts need to compare the accuracy of their method to this widely established benchmark. Forecasting practitioners should favor simple averages over more complex methods unless there is strong evidence in support of differential weights. provided helpful suggestions. Using the Structured-case Approach to Build Theory in E-Government

The paper concludes that the structured-case approach demonstrates the process of knowledge and theory building because it clearly points out the linkage between the data collected and the conclusions drawn. Hence, the application of the structured-case approach assures scientific rigor-an element that is often missed or done poorly in IS case study research.
