Prevalence and Determinants of Medication Adherence among Patients with HIV/AIDS in Southern Vietnam

This study was conducted to determine the prevalence and determinants of medication adherence among patients with HIV/AIDS in southern Vietnam. Methods: A cross-sectional study was conducted in a hospital in southern Vietnam from June to December 2019 on patients who began antiretroviral therapy (ART) for at least 6 months. more » ... up visits as scheduled. Multivariable logistic regression was used to identify determinants of adherence. Key findings: A total of 350 patients (from 861 medical records) were eligible for the study. The majority of patients were male (62.9%), and the dominant age group (≥35 years old) accounted for 53.7% of patients. Sexual intercourse was the primary route of transmission of HIV (95.1%). The proportions of participants who took the correct medicine and at a proper dose were 98.3% and 86.3%, respectively. In total, 94.9% of participants took medicine appropriately in combination with food and beverage, and 75.7% of participants were strictly adherent to ART. The factors marital status (odds ratio (OR) = 2.54; 95%CI = 1.51–4.28), being away from home (OR = 1.7; 95%CI = 1.03–2.78), substance abuse (OR = 2.7; 95%CI = 1.44–5.05), general knowledge about ART (OR = 2.75; 95%CI = 1.67–4.53), stopping medication after improvement (OR = 4.16; 95%CI = 2.29–7.56) and self-assessment of therapy adherence (OR = 9.83; 95%CI = 5.44–17.77) were significantly associated with patients' adherence. Conclusions: Three-quarters of patients were adherent to ART. Researchers should consider these determinants of adherence in developing interventions in further studies.

P atient 1. A 48-year-old male construction worker, with no preceding medical illnesses, was admitted to the intensive care unit of a hospital in Hanoi. For a few days prior to admission he experienced pain and numbness in both forearms and a flushed sensation throughout his body. He also reported increased perspiration and increased libido. At presentation, the patient was lucid but markedly agitated, and was unable to swallow due to involuntary inspiratory muscle spasms when he was presented
more » ... ith a glass of water (Video S1) or when he felt a breeze. On examination he was afebrile, and had a dry mouth, normal heart rate and blood pressure, and a Glasgow Coma Score of 15. He had no focal neurological signs and no neck stiffness, and Kernig's sign was negative. Neither the patient nor his wife recalled that he had been bitten by a dog, cat, bat, or other mammal in the preceding months, and his skin showed no evidence of recent bite injuries or cuts. Patient 2. A 37-year-old male farmer, without any prior medical history, presented himself to a Hanoi outpatient clinic with increased perspiration, intermittent muscle spasms of both legs, and the inability to drink normally due to involuntary inspiratory muscle spasms when he was presented with a glass of water or felt a breeze. Disease onset was several days before admission. On examination, the patient had a normal body temperature, heart rate, and blood pressure, and a Glasgow Coma Score of 15. The patient was lucid with periods of agitation. There were no focal neurological signs and no neck stiffness, and Kernig's sign was negative. He had bilateral pupil dilatation (~5 mm) with weak direct and consensual light reflexes. There were no signs of recent bites or other skin injuries, but the patient reported that he had been bitten in the heel by a pet dog one month prior to presentation. The dog had remained healthy since the bite. The families of the patients in this manuscript have given written informed consent to publication of their case details.

Preferences for methadone clinics among drug users in Vietnam: a comparison between public and private models

Methadone maintenance treatment (MMT) has been proven to be effective in treating opioid dependence. In Vietnam, MMT services are provided primarily by public clinics, with only one private MMT clinic established in recent years. Assessing the preferences of patients for different MMT models is important in evaluating the feasibility of these models. This study measured the preferences of drug users enrolling in public and private MMT clinics in Vietnam and examines the related factors of these
more » ... preferences. A cross-sectional study was performed on 395 participants at 3 methadone clinics in Nam Dinh. Data about the preferences for MMT models and sociodemographic characteristics of participants were collected. Exploratory factor analysis was employed to explore the construct validity of the questionnaire. The chi-square test and Mann-Whitney test were used for analyzing demographic characteristics and preferences of participants. Multivariate logistic regression identified factors associated with participants' preferences. Half the participants received MMT treatment in a private facility (49.4%). Two preference dimensions were defined as "Availability and convenience of service" and "Competencies of clinic and health professionals". Self-employed patients were more likely to consider these two dimensions when choosing MMT models. Only 9.9% of participants chose "Privacy" as one of the evaluation criteria for an MMT facility. Compared to public clinics, a statistically higher percentage of patients in the private clinic chose the attitudes of health workers as the reason for using MMT service (34.7% and 7.6% respectively). Mean score of satisfaction towards MMT services was 8.6 (SD = 1.0), and this score was statistically higher in a public facility, compared to the private facility (8.7 and 8.4 respectively). The study highlighted patterns of patient preferences towards MMT clinics. Compared to the public MMT model, the private MMT model may need to enhance their services to improve patient satisfaction.
